Community Care Access Centres (CCACs)
provide a single point of access to health and personal support services to help people live independently in their homes or to facilitate their transition into a long-term care home. Assisting people of all ages and cultures, whatever the diagnosis, CCACs plan, coordinate and ensure the delivery of a full-range of in-home and community-based services to support clients and their caregivers.
Since 1997, CCACs throughout Ontario have provided quality care within a client-centred philosophy and helped thousands of people by arranging for the delivery of health support services in their homes, schools, work environments, community health centres and shelters. Today, community-based care is the fastest growing area of health care in Canada.
The CCACs of Ontario offer health care professionals the opportunity to join the front lines of community care and make a real difference in people's lives. We provide services in all official languages, and offer competitive salaries, impressive benefits and continuing education initiatives. Working with us, you'll get the chance to use creativity and autonomy in decision-making, and most importantly, you'll get to see the results of your efforts every step of the way
