Community Care Access Centres (CCACs) provide a single point of access to health and personal support services to help people live independently in their homes or to facilitate their transition into a long-term care home. Assisting people of all ages and cultures, whatever the diagnosis, CCACs plan, coordinate and ensure the delivery of a full-range of in-home and community-based services to support clients and their caregivers.
Since 1997, CCACs throughout Ontario have provided quality care within a client-centred philosophy and helped thousands of people by arranging for the delivery of health support services in their homes, schools, work environments, community health centres and shelters. Today, community-based care is the fastest growing area of health care in Canada.
