POSITION SUMMARY

The incumbent will work with a team of Coordinators and Team Assistants in the Client Service Centre to provide service to clients (new or existing), referral sources and service providers who require timely assistance from the Toronto Central Community Care Access Centre.  This will include conducting telephone and occasional face to face assessments and the development of service plans for new clients who require immediate assistance as well as assisting clients who have information and referral needs.  You will be responsible for determining eligibility, explaining service options, helping clients and their caregivers to identify care needs, establishing and maintaining strong ties with the community and advocating for clients on urgent issues related to eligibility and service equality.  You will be responsible for working with clients and caregivers to ensure that there needs are met through the development, implementation, monitoring and evaluation of service plans.

You will also be responsible for providing support to current clients and service providers who require timely assistance to address emerging client service needs or issues.  The successful incumbents will be effective in the development of innovative and responsive service options to meet client needs.  The incumbents will also be responsible for researching and developing information and referral resources for the staff and clients of the Toronto Central Community Care Access Centre and will be a key resource for effective service delivery.  The role involves significant contact with clients, their families, caregivers, service providers, co-workers and community agencies.  Working in accordance with a service-oriented model, you will spend the majority of your time at Toronto Central Community Care Access Centre office.

The incumbent will primarily work evening and weekend shifts for a total of 15 hours (shifts include 8:30am to 4:30pm, 10:00am to 6:00pm, 4:00pm to 8:00pm & 12:00pm to 8:00pm or other variations Monday to Sunday as required).

QUALIFICATIONS

1.       A Nurse, Physiotherapist, Occupational Therapist, Speech Language Pathologist or Social Worker (Master of Social Work) currently licensed, registered or certified according to the requirements of the profession in the Province of Ontario

2.       Demonstrated excellent interpersonal, communication, decision-making, assessment skills, and high flexibility is required.

3.       Ability to work independently and co-operatively in a busy multi-disciplinary situation

4.       Demonstrated computer literacy in a Windows environment (Work and Excel) is required

5.       Knowledge of community resources

6.       Fluency in other languages is an asset
