The Community Care Access Centres are dedicated to supporting and enhancing the quality of life, independence, health and well-being of individuals in the communities we serve by offering a single point of access for community services and demonstrating leadership and excellence in community care.


The successful incumbent will assess referred clients for eligibility and assist ineligible clients in finding alternative sources of care.  The candidate will be responsible for coordinating delivery of health care services and the monitoring of both health care services and continuing eligibility of clients.  


Minimum Qualifications & Position Requirements:

·         A nurse with a relevant related degree/diploma, therapist or social worker with a degree, currently licensed, registered, or certified according to the requirements of the profession in the Province of Ontario.

·         A minimum of two (2) years coordination/case management/community health or related clinical experience

·         Demonstrated excellent interpersonal, communication, and decision making ability

·         Ability to work independently and co-operatively in a busy multi-disciplinary situation

·         Flexibility in adjusting to various work hours according to need

·         A valid driver's license and a reliable vehicle are required

·         Assumes other duties in the access centre as appropriate

·         The successful applicant will be required to demonstrate proficiency in all applicable computer programs

·         Ability to lift up to 20 lbs

 

Preferred qualifications:

 

·         Recent hospital experience and adequate knowledge of community resources

·         Fluency in a second language is an asset
