Geographically, Champlain is one of the largest CCACs, delivering care to over 20,000 clients everyday. Our region includes Eastern Counties, Renfrew County, and a portion of Lanark, Leeds & Grenville, spanning a diverse demographic population of both rural and urban clients.

WE ARE CURRENTLY SEEKING FULL-TIME, PART-TIME AND CASUAL CASE MANAGERS

As the focal point by which clients access as range of services, the Case Manager collaborates with clients, families, caregivers or substitute decision makers to assist clients in achieving their short and long term health care goals.

Case Managers are committed to:

. Assessing referred clients for admission and establishing a helping relationship with eligible clients and their families.
. Goal setting and service planning by establishing a care and service plan that balances client's needs, choices and available resources.
. Accessing resources by collaborating and partnering with networks of service providers.
. Initiating the implementation of the care plan.
. Reviewing, monitoring, and adjusting the care plan in compliance with quality standards and outcome measures.
. Conducting discharges from the service process and/or redirecting clients and families to alternative resources.

To join our dedicated team of professionals as a Case Manager, you have:
. Degree in Nursing (with current Certificate of Competence from the Ontario College of Nurses), OR
Degree or diploma in Physiotherapy (registered with the College of Physiotherapy of Ontario), OR
Degree or diploma in Occupational Therapy (registered with the College of Occupational Therapy of Ontario) OR
Degree in Social Work (registered with the Ontario College of Social Workers and Social Service Workers)
and a minimum of 4 years related professional experience OR
Registered Nurse (with current Certificate of Competence from the Ontario College of Nurses) and a minimum of 5 years of professional experience
. Professional experience in health care including linking with other community health agencies, needs assessment, case management and/or discharge planning

Your knowledge base and skill set includes:
. Client centred, multi-disciplinary team approach to client care
. Understand and embrace opportunities for improvement
. Demonstrates initiative and understands "team dynamics" vs. "team player"
. Effective decision-making and problem-solving in crisis situations
. Ability to communicate effectively orally and in writing
. Positive teamwork with ability to accept and adapt to change
