Case Manager
Permanent Full-Time
As a Case Manager, you will act as a liaison between your clients and the various health care providers needed to achieve their optimal health, independence and dignity. Drawing on your knowledge of health services and your well developed assessment skills, you will help your clients navigate through the complexities of the health care environment. This will involve explaining relevant services and coordinating and monitoring their implementation. You will be there to facilitate your clients' health care experience, linking them with the right information and helping them understand and manage their short and long-term health care goals.
If you are a Registered Nurse, Occupational Therapist, Physiotherapist, Speech Language Pathologist or Certified Social Worker (MSW only) in Ontario, we'd like you to become part of our team. SUMMARY OF ESSENTIAL QUALIFICATIONS
- BScN, BScPT, BScOT, MSW plus a minimum of one (1) year of relevant experience in a community health setting
- Nursing Diploma plus a minimum of ten (10) years of relevant experience in a community health setting
- Member in good standing with a relevant regulatory body in Ontario - as a Registered Nurse, Occupational Therapist, Physiotherapist, Speech Language Pathologist or Certified Social Worker (MSW only)
- Excellent interpersonal, communication, decision-making and assessment skills
- Ability to work independently and co-operatively in a busy multi-disciplinary situation
- Demonstrated computer literacy is required, as laptop usage during home visits is an essential part of the job
- A valid driver's license and a car is required for home visits
ASSETS
- Case Management Certificate is an asset
- Knowledge of community organizations and resources
- Experience working with diverse, multicultural client groups (including homeless, acquired brain injury (ABI), pediatrics, etc)
- Fluency in languages other than English
HOURS OF WORK
- Days, evenings, weekends
- Must be available for a 5 week orientation
