We are one of Canada's largest regional acute care hospitals, serving a catchment area of more than 850,000 people in the northwest GTA. The hospital currently operates on three sites with a total of 549 beds, 3,000 staff, approximately 700 physicians and 400 volunteers.

An immediate opportunity exists for a Clinical Nurse Specialist (CNS) position in the Bariatric Program. Reporting to the Program Director- Surgery/ Bariatric Program, the CNS is responsible for the coordination of the patient flow along the continuum of care for bariatric surgical patients. The CNS ensures the delivery of safe quality care to all patients/families in the bariatric program in collaboration with the multidisciplinary team.  The CNS  promotes, fosters and leads by example, a culture in which all staff are prepared to manage bariatric surgical patients safely and efficiently, through assessments, education, and multidisciplinary team collaboration along with quality patient care based on best practice and evidence-based guidelines from the referral intake to post- discharge follow up. The CNS plays a leadership and change agent role in the advancement of best practice and standards-   emphasizing patient safety, implementation of best practices in the care of bariatric patients and performing quality improvement  and coordination of responsibilities.

 Responsibilities include:
. Provides leadership in the coordination of quality care initiatives through the entire continuum of care for bariatric surgical patients
. Works closely with the Medical Director of Bariatrics in the pre-screening,  selection and prioritization/ triaging of appropriate candidates for bariatric surgery  
. Participates in Program Planning in conjunction with directives from the Ontario Bariatric Network and U of T Bariatric Collaborative Partnership Network
. Collaborates with  multidisciplinary team members of the bariatric team in the discussion, planning and recommendations of care treatment and strategies  for bariatric surgical patients
. Attends the Bariatric Team Rounds to discuss bariatric patients and appropriateness to proceed with the program 
. Ensures implementation of care initiatives and processes throughout all phases of the patient care flow for bariatric surgical patients
. Quality monitoring of process and patient outcomes in the Bariatric Program
. Attends the Bariatric Program Steering Committee and U of T Collaborative Clinical Committee

The position requires:
. Current Ontario Registration as a Registered Nurse in good standing
. Master's degree in Nursing with acute medical-surgical care experience
. Certified Diabetes Educator credentialing (preferred)
. At least two years of previous clinical experience with acute medical-surgical patients 
. Demonstrated knowledge of the care and management of bariatric surgical patients
. Demonstrated knowledge of  quality monitoring of patient outcomes related to bariatric surgery  (including coordination, review, evaluation of data and making recommendations for implementation of practice changes to improve patient outcomes)
. Demonstrated ability to be flexible and work effectively in high-pressured clinical situations and in a busy environment with changing priorities
. Excellent organizational, interpersonal and communication skills
. Effective time management and independent problem-solving skills
. Knowledge and demonstrated ability in adult teaching and learning principles required.  Completed courses in adult education preferred.
. Ability to work effectively in a diverse team environment
. Demonstrated commitment to ongoing professional development
. Knowledge of a second language an asset
