The Programs and Partnership Division (PPD) of the Centre for Communicable Diseases and Infection Control (CCDIC) is seeking the services of a SURVEY DEVELOPER to continue working on existing questionnaires on FluidSurveys (FS) for collecting and monitoring performance measurement data projects.

Role and Responsibilities

As of March 31,2013, the Agency discontinued its use of PDCAS to gather Grants and Contributions monitoring and performance measurement (PM) data. CCDIC and the Agency's regional offices used PDCAS to gather this data from community-based organizations under the Federal Initiative to Address HIV/AIDS in Canada's AIDS Community Action Program and under the Hepatitis C Prevention and Research Program. Currently, however, all units involved in PM data collection

CCIDC intends to rely on FS to support data-gathering and allow the program to report on the information collected from recipients in a format approved by the Agency (Excel or SPSS). Fully tested
and implemented surveys must be ready to be rolled out by March 31, 2014, to meet the next reporting
period, starting April 1, 2014.

� Develop, publish, and deploy new surveys based on the existing FS questionnaires and Excel questionnaires (Excel templates) using FluidSurveys® software.

â?¢ Adapt the existing FS questionnaires based on the changes the Excel templates undergo and the Standard Operating Procedures (SOPs) for the questionnaires so that they reflect the new tool.

â?¢ Develop training materials for funded recipients and for program consultants who will provide ongoing assistance to the recipients regarding FluidSurveys questionnaires;

Provide Webinar training to funded recipients (training to be offered on at least 3 instances);

Develop additional SOPs and documents to support future in-house Fluid Survey development and training needs.

Qualifications and Experience

Certificate, diploma, or degree in computer science, web design, or other relevant fields, from a
recognized post-secondary institution, is required;

Minimum of five (5) years of designing, coding, publishing, and deploying questionnaires using electronic
web-based surveys (e.g FluidSurveys) is required:
Asset: experience designing and coding FluidSurveys, specifically FluidSurveys5 (the latest version).

Other experience (not mandatory)
â?¢ Experience in writing Guidelines, Standard Operating Procedures (SOP), and Questions and Answers;
â?¢ Experience producing training materials for distance training provisions (eg. webinars)
