The BN prepared nurse provides nursing consultation services and advice to the staff on a wide variety of nursing issues related to the care and treatment of adults and older adult clients. This includes but is not limited to the provision of professional advice and recommendations on client cases presented at Interdisciplinary Team (IDT) meeting and case conferences.
DUTIES AND RESPONSIBILITIES
. Use a computer for client documentation in a Windows environment, including word processing, electronic mail and Internet.
. Request nursing assessments and reports from nurses and other health professionals; to review and finalize these reports and to follow-up with the respective staff of the IDT or others on services for the client; and to monitor, report on, and follow up on the Due Dates of forms assigned to external providers. Will be required to request, review and summarize nursing and related health professional reports from other community agencies and services.
. Participate in IDT meetings and case conferences on client or health-related issues. Make follow up calls and inquiries from a nursing perspective related to clients discussed at IDT meetings or during case conferences/consultations with staff.
. Liaise with health care professionals and agencies in the community and in long-term care facilities in relation to specific client cases.
. Make recommendations and give professional approval or decline of various services in accordance with policy, including determining the assessment instruments to be used, the related amount of time required, and advising the external providers of the professional recommendations and/or approval.
. Complete nursing assessments, visits and reports on clients in their homes or the facilities in which the clients reside. Prepare reports on the health status and care needs of the clients. Elements to be assessed include but are not limited to: Nursing Assessment, Mini-Mental Status, Depression, Pain.
. Apply competency in interpersonal skills, judgment, dependability, and oral and written communications.
. Perform duties in accordance with company policies and procedures and Standards of Nursing Practice as defined by the provincial regulating body.
. Understands and implements infection prevention practices.
. Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
. Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
. Complete other tasks as requested.
Graduation from an accredited School of Nursing with a bachelor degree in nursing and current registration with the CRNM.
At least two years of recent nursing experience, preferably in a community setting. Mental Health experience an asset.

Proven ability to effectively communicate with all levels of clinical and non-clinical Field employees and interpret company policies; strong track record in health teaching. Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format; ability to work independently and as part of a team; able to perform and prioritize multiple functions and tasks; evidence of current CPR certification; Child and Adult Abuse registry and Criminal Record Check. Must hold or be willing to obtain Canadian Security Clearance: a valid driver's license.
