Clinical Director, Integrated Care Solutions

Healthcare Services and Wellness

. Masters Degree in Health or Business Administration, Nursing or Health Informatics, or equivalent combination of education and experience
. Current Certificate of Registration from the College of Nurses of Ontario (CNO) Advanced education in clinical informatics or computer related field preferred Eight (8) to ten (10) years of progressive management experience in health care environment with preference in hospital and community home care
. Demonstrated knowledge of and experience in redesigning clinical processes to improve clinical practice, efficiency and patient care, preferably in conjunction with the implementation of a clinical information system
. Demonstrated leader of leaders; capable of developing other leaders to their full potential
. Demonstrated track record of delivering results with excellent financial and quality management, project management and risk management experience
. Fluent and computer literate with computer systems such as email and Microsoft Office applications (MS Word, Excel & PowerPoint)
. Demonstrated knowledge of and commitment to patient and staff safety
. Shared leadership skills with the ability to foster collaboration and shared direction among diverse clinical and non-clinical groups
. Exceptional analytical and information-seeking skills that contribute to effective decision-making and strategic planning and execution of action plans that deliver results and motivate individuals for greater performance excellence
. Experience in leading teams through pre- and post-evaluation exercises to deliver increased value through process change and clinical transformation events
. Track record for creating a quality and safety culture and means for monitoring and communicating results that is meaningful to team members and inspires continuous improvement
. Self-directed, courageous, and highly motivated with excellent interpersonal and communication skills
. Experience managing competing demands in a fast-paced, multi-site, multi-disciplinary health care environment
. Ability to work with a diverse group of leaders and stakeholders with demonstrated success in moving key stakeholders to positive resolution and to facilitate and coach clinical areas on change initiatives
. Demonstrated track record as a highly regarded contributor within a management team


DUTIES AND RESPONSIBILITIES:

 Lead clinical projects identified by the Director of Integrated Care
 Conduct clinical analysis and research on best practice and clinical issues related to integrated models of care
 Active leadership role in the design, implementation, adoption and sustainability of integrated digital health systems (e.g. electronic medical records, portals, etc.), for professionals and paraprofessionals, resulting in greater efficiency in the utilization of digital health systems for the delivery of safe, high quality evidence-based care.
 Active leadership role in the design, implementation, adoption and sustainability of a operating platform / system to support bundled funding and bundled care models
 Lead the clinical development and implementation of new programs with hospitals and community partners
 Lead or assist with the integration of client satisfaction initiatives; ensure that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner
 Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body
 Liaise with other Case Managers, hospitals, funders or government programs as required
 Liaise with Provincial regulating body as required
 Participate in business development as appropriate; assist with the development of contractual agreements for client care and service as requested.
 Participate in strategic planning and market analysis to achieve growth goals.
 Collaboration with Business Development Manager and/or Director on key and shared accounts
 Attend and participate in sales, operational or external meetings as requested
 Promote Bayshore Home Health through participation in local healthcare or business committees as well as community events
 Participate in ongoing internal and/or external continuing education activities
 Adhere to Bayshore Policies and Procedures
 Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System
 Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns. Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel
 Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal
 Complete other tasks as requested
