Hours: up to 18.75 hours per week
Status: Temporary Part Time (up to 12 months)
Toronto Western Family Health Team - Garrison Creek Site



The Family Health Team Pharmacotherapy Specialist is an integral member of the Pharmacy department and the Family Health Team. A key player to advance the pharmacist's role in the clinical care of patients as well as increased patient safety, teaching of pharmacy residents, Pharm D students and mentoring other staff members. The FHT Pharmacotherapy Specialist will work with the FHT and the Pharmacy Department to establish guidelines and provide implementation strategies for initiatives including: education to clinical hospital staff, prospective and retrospective evaluation of ongoing pharmacist activities, and provide feedback. As a leader, you will be responsible for providing education and feedback to students, pharmacists and other members of the healthcare team regarding patient care, therapeutics, teaching and research initiatives.


Clinical Practice (80%)

. Conducts comprehensive assessments for individual patients to identify actual and potential drug therapy issues. Formulates comprehensive care plans which incorporate individual patient preferences and goals.
. Acts as a clinical pharmacy resource for internal and external health care professionals in a patient-focused practice
. Work within and Inter-professional team to promote rational evidence based prescribing to optimize patient outcomes within the inter-professional teams
. Provide clinical direction on complex cases related to their clinical practice
. Provides teaching on pharmacotherapy to the FHT Staff, trainees, and other Allied Health Staff:

o Participates in, and / or leads educational sessions for staff within the FHT and Pharmacy Department
o Participates in, and / or leads inter-professional teaching sessions
o Involved in teaching and mentoring of Pharmacy Residents

. Establishes initiatives that foster improved clinical pharmacist activities
. Establishes initiatives that increase patient safety and promote cost savings
. Participates in the development of evidence based practice treatment guidelines
. Manages patients on warfarin through a medical directive
. Participates in the implementation of the guidelines including:

o Education
o Audit and feedback on usage
o Feedback on patient risk reduction and outcomes as appropriate
o Patient Safety
o Provides Drug Information
o Patient specific recommendations
o Inter-professional consults

Quality Improvement/ Research/ Projects (12.5%)

. Participates and leads quality improvement initiatives with the goal of improving patient safety
. Identifies opportunities for pharmacy-based and inter-professional research within his/her area of practice and specialization
. Develops hypotheses and research processes, oversees and supports ongoing research activities and/or coordinates the involvement of research assistants, students, pharmacy residents and pharmacists in research activities
. Collaborates with interdisciplinary teams to conduct pharmacotherapy -based research
. Provides supervision and mentoring for pharmacy students, residents and fellows research projects
. Publishes in peer reviewed journal
. Represents the hospital and the University at provincial, national and international meetings
. Participates in, and/ or leads quality improvement initiatives which address the 6 domains of quality in healthcare (Patient Centered, Safety, Timely and accessible, Effective, Efficient, Equitable)
. Participates in and / or Leads Patient Safety Rounds
. Participates in the incident reporting system

Service (7.5%)

. Represents the Department of Pharmacy Services, The Family Health Team and/ or the University (Faculty of Pharmacy or Medicine) on inter-professional care committees, and standing hospital committees
. May be required to participate in conferences and regional planning groups as it relates to their clinical practice area

. Has the ability to work on complex problems with various clinical and technical stakeholders to provide innovative and creative solutions to achieve resolution and maintain patient safety
. Demonstrates leadership skills
. Practice experience in the clinical specialty area

. Knowledge of research, statistical and analytical methodologies

. Completion of a Bachelor of Science Degree in Pharmacy from an accredited program of study or recognized equivalent required
. Completion of a post graduate Pharmacy degree ( e.g. clinical Master's degree or Doctor of Pharmacy) preferred
. Completion of a pharmacy residency program preferred
. Completion of a research fellowship an asset


. Pharmacy practice in related areas of patient focused or population health preferred
. At minimum four (4) to six (6) years practical or related experience
. Three (3) to five (5) years of hospital experience
. Experience with primary or community care an asset
. Previous independent research experience preferred


. Certificate of Qualification with the Pharmacy Examining Board of Canada required
. Current registration with the Ontario College of Pharmacists required
