UniversalCare Health Services, provides therapy services to Long Term Care homes. We are currently recruiting an OT to support residents in Long Term Care homes located in York Region. The Occupational Therapist is a member in good standing of the College of Occupational Therapists of Ontario*
- With previous work experience in long-term care setting.
- Excellent verbal communication skills
OTHER REQUIREMENTS
A qualified staff will be screened for tuberculosis and required to conduct a criminal reference check including a vulnerable sector screen prior to commencing in a LTC home.
RESPONSIBILITIES
Clinical Care
- Upon receipt of referral, to provide clinical care for assigned residents independently. This includes the following:
- Assessment and re-assessment related to the role of occupational therapy in long-term care setting;
- Treatment - individual and group program(s) related to the role of occupational therapy in long- term care setting;
- Supervising the assistant(s) and support personnel;
- Discharge planning.
Other duties
- Providing in-services and various educational seminars including nursing rehab/restorative programs to staff of nursing home;
- Providing input/report for multidisciplinary care conferences;
- Documentation;
- Participating clinical team planning;
- Liaising with vendors (preferred by nursing home and/or chosen by resident's POA and family) to efficiently expedite the delivery and maintenance of equipment whilst keeping the clinical team informed;
- Providing leadership and attending various committees including restraint reduction, fall prevention, health and safety and professional advisory etc.
Communication
- Ongoing communication between physiotherapist(s) and other health care providers;
- Ongoing communication with residents and/or their family members;
- Maintaining amicable relations with residents, family members, and nursing home staff;
- Excellent interpersonal, verbal communication and customer service skills relating to the needs and challenges of residents and their families;
- Legible and professional written documentation skills;
- Awareness to exercise confidentiality pertaining to the nursing home and resident information
