Job Description
Accountabilities:

Work collaboratively with the Pharmacy Team to provide patient-centred care
-
Dispensary duties including medication order entry and medication profile review
-
Provide drug information to physicians, nurses, and other clinicians to enhance patient care
-
Verification of all medication orders for inpatients
-
Provide therapeutic drug monitoring
-
Participate on hospital committees as required
-
Perform and/or supervise in patient dispensing (which includes computerized order entry, unit dose, centralized IV admixture services, TPN, Chemotherapy and an understanding of automated systems) drug information and administrative activities as assigned

Resolve day-to-day clinical issues

Assist with the implementation of pharmacy initiatives, which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operations

Other duties as assigned
Qualifications
-
Currently licensed (or eligible) by the Ontario College of Pharmacists, Part A of the registry
-
Relevant clinical experience preferable
-
Excellent attendance, punctuality and work record
-
May be required to travel between and work at all Osler sites
-
Excellent computer skills
-
Excellent oral, interpersonal, and written skills
-
Demonstrate initiative and a willingness to work collaboratively in a team environment
-
Strong clinical assessment skills in identifying and resolving drug-related problems
-
Demonstrated ability to apply clinical judgment in analyzing and solving clinical problems
-
Meditech computer experience preferable

Must be able to meet the physical demands of the position

Additional Information
Hours: Nights 12, including weekends and statutory holidays (subject to change in accordance with operational requirements)
