The Director of the pharmacy is currently seeking a part-time Registered Pharmacist. The purpose of the position is to provide professional pharmaceutical services to a psychiatric hospital including medication reconciliation and monitoring of drug therapy problems; to provide related consultative and education services and to assist in coordinating the activities of the pharmacy. Perform all work in compliance with professional practice standards consistent with the Regulated Health Professions Act (RHPA), the Pharmacy Act and the regulations.
KEY ACCOUNTABILITIES:

Complete medication reconciliation upon admission, transfer and discharge by creating the Best Possible Medication History (BPMH) utilizing medication from all sources
- Utilize the medication profile, patient, and medication-related information consistent with the principles of pharmaceutical care to:
- Assess prescribed medications; Monitor adherence to medication regiment;
- Identify and assess the type and significance of medication-related problems, and discuss with the physician and/or care team;
- Make appropriate interventions; Dialogue with the patient/family as appropriate; Document in a readily retrievable format any interventions and/or dialogue.

Computer Provider Order Entry (CPOE) and verification of prescriptions in the MEDITECH system,
- Providing drug information and education to Providers, nurses, other health care professionals and to clients and their families/caregivers;
- Ensuring proper functioning of the drug distribution system including procurement, storage, control and disposal of drugs, and maintenance of computerized inventory systems to ensure sufficient stock on hand;
- Ordering and receiving; recommending changes in inventory control or storage procedures; and performing/supervising inspections throughout the Centre

Investigating and reporting on adverse drug reactions, medication errors and drug utilization, noting significant trends.
- Providing technical supervision to Pharmacy Technicians, ensuring strict adherence to all regulations

Updating patients' medical profiles, maintaining prescription and client information records according to Ministry and legislated retention schedules, documenting clinical activities on the clients’ clinical record, establishing, maintaining and controlling narcotic and controlled-drug inventory and the drug information database as they relate to distribution, procurement and disposal.
- Maintenance of a variety of pharmacy records
- Participate in the University of Toronto, Faculty of Pharmacy, Early Student Exposure Program, Structured Practical Experience Program (SPEP), Ontario College of Pharmacists, training and internship programs and in pharmacy-related and collaborative interdisciplinary research.
REQUIREMENTS/QUALIFICATIONS:

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
- Licensed as a Pharmacist in Ontario.

Current knowledge of drugs and medical practices, dispensing, compounding, repackaging, drug monitoring, and drug utilization reviews to provide the full range of pharmaceutical services to a large psychiatric facility.
