This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.

Responsibilities

The Placement Coordinator is responsible for supporting the process of Placement in a Long Term Care Facility on behalf of the Nova Scotia Health Authority. The Placement Coordinator liaises with home care, acute care, and long term care facilities, receives long term care assessments for clients who are referred for Placement, builds on the long term care assessments with information required to determine whether people who have been referred for placement are appropriate for a long term care facility, communicates with client/Substitute Decision Maker on placement issues, manages the waitlist for Long Term Care to ensure that clients are admitted to long term care in an equitable fashion per the Department of Health and Wellness policy, coordinates admissions to Long Term Care facilities; coordinates transfers of residents between facilities; maintains records and provides written and statistical reports as identified by the Nova Scotia Health Authority. The Placement Coordinator communicates to staff in acute care, home care, long term care on placement and other stakeholders as required. The Placement Coordinator assists in managing and booking client stays in licensed respite beds.

Qualifications
Baccalaureate degree in a health related field is required
Current licensure with applicable professional designation
Minimum 3- 5 years clinical experience
Knowledge of the health system
Experience in long term care assessment/placement is an asset
Two- three years’ experience in the Continuing Care environment preferred
Strong client focus and ability to build solid client relationships
Demonstrates excellent written and oral communication skills
Demonstrates competence in the use of the Windows operating system
Ability to prepare correspondence using Microsoft Word or Outlook
Demonstrates competence in the use of Microsoft Excel
Excellent judgement and initiative required
Excellent planning, organizational and time management skills
Demonstrates conflict resolution skills, strong facilitation skills and excellent negotiation skills
Demonstrated collaboration, team work and leadership skills
Commitment to promoting a culture that supports safety, ethical practices and organizational health
Competencies in other languages an assest; French preferred
Hours of Work
Permanent Full-time Position; 70 hours bi-weekly
Salary Information

$31.8524 - $40.2222 hourly
