About the Role
CareOnDemand Home Health Care & Staffing Services is seeking an experienced Registered Nurse (RN) to join our growing rehabilitation and community care team. The successful candidate will conduct comprehensive in-home assessments for injured workers and community clients, develop care recommendations, coordinate services, monitor progress, and complete reassessments and discharge evaluations.
This role is ideal for an RN with experience in community nursing, rehabilitation, case management, occupational health, WSIB, or home and community care.
Key Responsibilities
- Conduct comprehensive in-home initial assessments of injured workers and community clients.
- Evaluate functional status, mobility, activities of daily living, safety risks, and support needs.
- Develop individualized care plans and service recommendations.
- Determine appropriate nursing, personal support, rehabilitation, and community support services.
- Complete scheduled reassessments to evaluate client progress and adjust recommendations as required.
- Conduct discharge assessments and provide outcome summaries upon completion of services.
- Collaborate with physicians, rehabilitation professionals, caregivers, and other members of the interdisciplinary team.
- Maintain accurate, timely, and professional clinical documentation and reports.
- Participate in care coordination and communication with referral sources, insurers, and community partners as appropriate.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Minimum 2 years of nursing experience.
- Community nursing, home care, rehabilitation, case management, occupational health, or WSIB experience preferred.
- Strong assessment, clinical reasoning, and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible caseload.
- Valid Ontario Driver's Licence and access to a reliable vehicle.
- Current CPR certification.
- Vulnerable Sector Check.
Preferred Qualifications
- Experience with WSIB, insurance-funded rehabilitation, or return-to-work programs.
- Experience completing clinical assessments and writing recommendations for ongoing services.
- Knowledge of rehabilitation principles, mobility assessments, and functional outcome measurement tools.

Pay: $45.00-$65.00 per hour
