Salary: $87,520 – $101,918 per annum depending on experience, pro-rated to FTE.
Hours of work: 20 hours per week (0.5 FTE), weekdays within the hours of 8am to 5pm, with flexibility to offer some late or early morning visits to meet patient preference.

The Georgian Bay Family Health Team is seeking a pharmacist to join our interprofessional team and support the delivery of comprehensive, patient-centred primary care. This is a new role within our Family Health Team (FHT). The successful candidate will be responsible for assessing the unique needs of the organization and leading the development, implementation, and evaluation of this program. As an integral member of the FHT, you will provide medication management services, chronic disease support, health promotion, and patient education to help improve health outcomes for our diverse patient population.
As the successful candidate, you will work collaboratively with family physicians, nurse practitioners, and other healthcare professionals to optimize medication therapy, support chronic disease management, and enhance the quality and safety of care delivered across our organization. Key responsibilities:
- Provide comprehensive medication assessments and consultations for patients with chronic and complex health conditions.
- Optimize medication therapy through patient assessments, medication reviews, reconciliation, and evidence-informed recommendations.
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary team members to support coordinated patient care.
- Participate in the planning, implementation, delivery, and evaluation of chronic disease management, health promotion, and illness prevention programs.
- Educate patients and families regarding medication use, adherence, self-management strategies, and healthy lifestyle practices.
- Foster safe medication practices through patient education and collaboration with healthcare providers.
- Provide drug information, prescribing guidance, and evidence-based updates to physicians and other team members.
- Conduct presentations and educational sessions for patients, healthcare professionals, and community partners as requested.
- Document assessments, recommendations, care plans, and medication profiles within the electronic medical record (EMR).
- Participate in quality improvement initiatives, case conferences, program development activities, and interdisciplinary care planning.
- Build strong relationships with patients, community pharmacists, healthcare providers, and partner organizations.
- Support access to care by conducting home visits when required.
Education and Qualifications:
- Registered and in good standing with the Ontario College of Pharmacists.
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD).
- Additional certification in pharmacotherapy, chronic disease management, diabetes education, geriatrics, or related specialty areas is considered an asset.
- Valid Ontario driver’s license and operational vehicle
Experience:
- Minimum three years of relevant pharmacy experience in a community, hospital, primary care, or combination setting.
- Understanding of primary care and community-based healthcare delivery.
- Experience working in an interdisciplinary team environment.
- Demonstrated experience in program development, implementation, and evaluation.
- Demonstrated ability to work independently while effectively managing multiple priorities.
- Use of Microsoft Office, PS Suite experience an asset
- Working autonomously and being self-directed
