COMMUNITY SUPPORT WORKER
TEMPORARY FULL-TIME (Approx. 12 Months)
POSITION OVERVIEW:
Under the supervision of the Director, Client and Stakeholder Relations, the Community Support Worker will provide support to older adults aged 55 + in Wood Buffalo, helping them to live independently in the community and improve their overall health outcomes. This position will provide a coordinated, person-centered approach, and help to address critical gaps in service navigation, social connection, and access to community-based supports.
This one-year, temporary role is specifically designed to address a significant service gap for adults aged 55–59, who are currently underserved during a critical transition period. Many people in this age range experience increasing health, social, and housing challenges without access to coordinated services. Without early intervention, these individuals frequently present at age 60 to organizations with unaddressed financial, housing, and support needs, increasing service complexity and placing additional strain on already capacity-limited programs.
The Community Support Worker will address this gap by providing early intervention, ensuring individuals aged 55–59 are connected to supports, informed about available services, and better prepared to transition into traditional seniors’ programs as they age.
RESPONSIBILITIES:
· Provide individualized, wrap-around support by conducting comprehensive client needs assessments
· Identify client goals and priorities
· Develop tailored service plans
· Connect clients to community-based supports like housing, transportation, financial benefits, social and recreational programs, in-home supports, and mental health resources
· Provide ongoing follow-up to ensure successful service access and adjust supports as needs evolve
· Work collaboratively with healthcare providers, social workers, and community organizations to enhance - not duplicate - existing services.
· Support professionals by addressing non-medical, health-related social needs, allowing clinical providers to focus on medical care
· Provide the connection for older adults to a range of social, community and partner support to enable them to improve their health and wellbeing and to help them age safely in place as they choose
· Assist in accessing food resources, transportation, physical activities, volunteer opportunities or assistance in filling out forms
· Maintaining a non-judgmental approach in every situation
· Practicing self-motivation and ability to work alone on a regular basis
· Understanding of professional boundary issues and working with people who have complex needs
· Assisting clients, as needed, to de-clutter, clean, and prepare for pest control
· Maintaining database of clients and their needs, as well as follow-up notes
· Helping clients to collect required documentation, including downloading, scanning, printing and uploading forms
· Conducting regular visits with clients
· Other duties as assigned
· EDUCATION REQUIREMENTS:
· Grade 12
· Post-secondary diploma in social work, human services or related disciplines required.
WORK EXPERIENCE REQUIREMENTS:
· Experience and knowledge of the Residential Tenancies Act, Health Act, Alberta Housing Act and the Alberta Building Codes preferred.
· Experience working with seniors and older adults
· At least 3 years’ experience and sensitivity regarding social issues that may be impacting daily living and housing stability
· Experience in case management
· Exemplary customer satisfaction skills, including dealing effectively with clients, co-workers, general public, and management team
· Excellent computer skills using Microsoft applications
· Effective communication, both written and oral
OTHER REQUIREMENTS:
· First Aid with CPR Training (or willing to obtain)
· Valid class 5 driver’s license with 3 years of driving experience and clean abstract
· Must have own vehicle
· Must provide a Criminal Record Check and a Vulnerable Sector Check for review and acceptance
WORKING CONDITIONS:
· Normal office conditions
· Travel within the urban service area is required and travel to the rural service area on an as needed basis.
· Evening and weekend hours as required.
SALARY:
$36.69 per hour **
