Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong understanding of medical procedures, excellent communication skills, and a commitment to improving patient outcomes. The LPN will work closely with interdisciplinary teams to deliver personalized care tailored to each patient's needs.
Duties
- Administer medications and treatments accurately according to physician orders and established protocols
- Monitor patient vital signs and assess patient conditions regularly
- Assist with patient admissions, discharges, and transfers
- Document patient care activities using electronic health records such as Epic or Cerner systems
- Provide wound care, including dressing changes and wound assessments
- Support patients with activities of daily living and mobility assistance
- Conduct phlebotomy procedures and laboratory specimen collection when necessary
- Educate patients and families about health management, medication use, and post-discharge care plans
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop comprehensive care plans
- Ensure compliance with all safety standards, infection control protocols, and regulatory requirements
Experience
- Prior experience in nursing home, hospital, or acute care settings preferred
- Very good knowledge of use the PCC system to make care plan
- Experience in dementia care, senior care, hospice care, or pediatrics is highly desirable
- Knowledge of medical terminology and medication administration procedures required
- Background in home care or assisted living environments is a plus
- Experience with pain management, case management, or laboratory procedures enhances candidacy
- Strong patient care skills combined with excellent communication abilities are essential for success in this role
