Position Summary
The Clinical Social Worker, Crisis and Care Coordinator provide comprehensive clinical, coordination, and crisis support to patients receiving psychiatric care through Northern Psychiatric Services. The role integrates patient care, referral management, clinic coordination, and collaboration with community and healthcare partners to ensure safe, timely, and culturally responsive psychiatric service delivery in northern communities. It will be delivered to a fast-paced practice in support of three psychiatrists that are serving the northern communities of La Loche, Île-à-la-Crosse, La Ronge, northernmost communities of Saskatchewan at Athabasca Health Region and catchment area. The position also provides case management, service coordination, and long-term, individualized support to individuals with complex needs who may be challenging to engage, including those living with severe and persistent mental illness, substance use disorders, acquired brain injury, and legal or social complexities. Ages of individuals would vary from youth to elderly. The work is both challenging and rewarding.
Education & Professional Registration
- Bachelor of Social Work (BSW)
- Registered member in good standing with the Saskatchewan Association of Social Workers (SASW)
- Experience: more than 5 Years in case management, housing support and community mental health. Experience in a crisis intervention role
- Competencies: Advanced analytical, computer, writing, verbal communication, interpersonal and organizational skills
- Knowledge of trauma-informed and cultural competent care will be an asset

Referral Management & Intake
- Receive, review, and process new referrals for psychiatric services
- Communicate with patients regarding referral status and appointment scheduling
- Liaise with Nurse Practitioners, Physicians, Community Mental Health Nurses (CMHN), Mental Health Workers and schools regarding:
- Coordinate psychiatric assessments for patients currently admitted to hospitals, prioritizing urgent openings when available
Clinic Coordination & Administrative Support
- Coordinate appointments and communication with community mental health supports (MH Workers, CMHN, etc.), including assisting patients who lack reliable phone access
- Work collaboratively with northern teams to connect individuals facing barriers such as transitional housing, unstable contact information, or limited supports
- Build and maintain positive working relationships with northern community partners to enhance patient support and continuity of care
- Manage and maintain clinic schedules within the EMR
- Coordinate clinic bookings with Northern Medical Services (NMS), including:
- Coordinate psychiatric teaching sessions for northern medical residents
- Provide monthly regional clinic statistics and reporting
- Coordinate resident clinic schedules, travel arrangements, and clinic setup
- Provide administrative and logistical support to residents as required
- Manage clinic-related faxes and emails
Patient & Family Care
- Act as a point of contact for patients between psychiatric appointments for:
- Provide supportive counselling and crisis intervention for patients and families during periods of acute distress
- Assist patients with applications and documentation related to:
- Liaise between physicians and patients to facilitate timely communication and care coordination
- Support patients in connecting with:
- Communicate with pharmacies regarding refill requests, missing prescriptions, medication concerns, and special access medications
- Provide longer-term follow-up and check-ins with patients as needed

Status: Term 12 moth with the possibility of extension or becoming permanent

Shift: Typically, regular business hours, but flexibility is required to accommodate client needs, including occasional evenings or weekends.

Salary: The salary range, based on 1.0 FTE, is $69,035.00 - 107,868.00 per annum. The starting salary will be commensurate with education and experience.
