About the Client:
We're seeking a compassionate caregiver for a 76-year-old woman in Bracebridge who is experiencing cognitive decline and managing type 1 diabetes. She lives with her adult son who has special needs. Her husband recently moved to a nursing home due to health complications, and her mother just entered palliative care, creating additional emotional stress. She has good days and bad days with memory and daily task completion, and experiences frequent urinary tract infections that worsen her cognitive symptoms. She is currently transitioning from an insulin pump to injectable insulin, which requires careful monitoring and reminders.
Responsibilities:
- Provide medication reminders and support her in self-administering insulin based on blood sugar readings using a provided guideline
- Prepare nutritious meals appropriate for diabetes management
- Perform light housekeeping and tidying to maintain a safe, comfortable living environment
- Offer memory prompts and gentle redirection for cognitive support
- Provide transportation for errands and medical appointments
- Monitor for signs of urinary tract infections or changes in cognitive function
- Engage her in meaningful activities and provide companionship
- Complete visit reports after each shift documenting observations and any concerns
- Communicate regularly with family members about her condition and any changes
What We're Looking For:
- PSW certification or equivalent caregiving experience
- Valid driver's license and reliable transportation
- Experience supporting individuals with cognitive impairment or memory loss
- Knowledge of type 1 diabetes management and blood sugar monitoring
- Ability to follow medication reminder protocols and assist with self-administration
- Strong meal preparation skills with understanding of diabetic-friendly nutrition
- Patient, calm demeanor with ability to provide gentle redirection
- Excellent communication skills and ability to recognize changes in condition
- Reliable, consistent, and takes initiative when needed
- Brings positive energy while respecting independence
- English speaking
Schedule:
Daily visits, Monday through Friday, approximately 10:00 AM start time, 2 hours per visit. This schedule may adjust based on assessment results from Ontario Health at Home services. Consistency is important as she benefits from routine and familiar faces.
This position offers the opportunity to make a meaningful difference in someone's life during a challenging transition period. You'll work alongside a dedicated care manager and have support from our nursing team who reviews care plans monthly. The family is actively involved and committed to ensuring their mother receives quality care that helps her maintain independence and dignity at home.
