We are currently seeking an experienced Clinical Referral Coordinator / Specialist Navigator to join our team. This position is ideal for a healthcare professional who is passionate about patient care, skilled in triage and referral coordination, and thrives in a fast-paced clinical setting.
Position Summary
The Clinical Referral Coordinator / Specialist Navigator serves as a vital link between patients, family physicians, specialists, and healthcare providers. This role is responsible for managing referrals, performing patient triage, coordinating appointments, maintaining accurate records, and ensuring patients receive timely access to specialty care.
Key Responsibilities
- Review, triage, prioritize, and process patient referrals.
- Coordinate specialist consultations, diagnostic testing, and follow-up appointments.
- Maintain accurate documentation within HealthQuest EMR.
- Act as the primary point of contact for patients regarding referral status and scheduling.
- Communicate effectively with physicians, specialists, Alberta Health Services (AHS), and community healthcare providers.
- Monitor referral progress and ensure timely completion of care pathways.
- Support patient navigation throughout the referral and treatment process.
- Maintain compliance with FOIP and healthcare privacy regulations.
- Assist with administrative duties related to patient care coordination and scheduling.
- Contribute to process improvements that enhance patient experience and clinic efficiency.
Mandatory QualificationsEducation & Certification
- Graduate of an accredited Nursing Program (LPN or RN).
- Medical Terminology Certificate.
- Valid FOIP (Freedom of Information and Protection of Privacy) Certificate.
- Current CPR certification is an asset.
Experience
- Minimum 2 years of healthcare experience in a medical clinic, specialist office, hospital, or healthcare setting.
- Minimum 2 years of patient triage experience.
- Mandatory experience using HealthQuest EMR.
- Experience managing referrals, patient scheduling, and care coordination.
- Cardiology or specialty clinic experience is considered a strong asset.
- Familiarity with Alberta Health Services (AHS) processes and Connect Care is an asset.
Language Requirements
Candidates must be fluent in English.
