Hours: 37.5 hours per week weekdays

JOB SUMMARY:
The Community Support Worker (CSW) is a member of the Lyndhurst Centre interdisciplinary inpatient team. The Community Support Worker’s focus is on supporting patients to transition to the community by connecting and advocating with patients to access various community based resources. These resources include, but are not limited to: income replacement; housing; equipment funding; identification replacement; and additional community resources, such as local food banks. Where resources are limited, the CSW will collaborate with patients and the inpatient team to assist in navigating all possible avenues of support needed by the patient. The CSW also provides educational information in order to support patients in making decisions that will best meet their individual needs and particular circumstances. The CSW participates in program planning and quality improvement activities; engages in program and hospital professional development; and collaborates with inter-professional teams as well as community-based and private service providers.
QUALIFICATIONS:
Social Service Worker diploma or an equivalent combination of education and experience
Completion of a Bachelor’s Degree in Social Work, Arts, Science or equivalent is an asset
Registration with the College of Social Workers and Social Service Workers is required
Experience navigating community resources and essential services
Computer proficiency
Knowledge of policies, and programs related to income insecurity and access to housing
Strong verbal and written communication, and advocacy skills
Knowledge and/ or experience of navigating Mental Health and Addiction resources is an asset
Previous experience working with the Spinal Cord Injury patient population and/or work experience in a hospital preferred
Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
