The Nurse Practitioner (NP) with Medicine Place Pharmacy Plus will provide advanced clinical care and comprehensive health services within a community-based practice and sometimes within a virtual environment. This role will be based two days a week at Medicine Place clinic and three days a week at an onsite partner clinic. The NP will focus on providing primary care for individuals with mental health and addictions challenges. This role requires the NP to work as a member of the multi-disciplinary team to ensure integrated, person-centered care. Responsibilities include conducting assessments, diagnostics, interventions, and ongoing management of individual care, monitoring responses to medications, adjusting treatment plans, and educating individuals on potential side effects and drug interactions. The NP will work closely with both community and Medicine Place health practitioners, using a holistic, culturally competent, and trauma-informed approach.
Responsibilities:
Client Engagement & Care Management:
- Conduct comprehensive assessments, including ordering and interpreting appropriate screening, diagnostic, and laboratory tests.
- Develop, implement, and evaluate individualized, client-centered care plans in collaboration with individuals, physicians, pharmacists, and other team members.
- Prescribe medications, monitor individual responses, adjust treatment plans as necessary, and manage treatment interventions.
- Provide virtual care where required
Documentation & Records:
- Maintain accurate and complete records of individual visits and interventions through timely documentation.
- Participate in initiatives to enhance healthcare support efficiencies and improve outcomes.
Education & Resource Development:
- Develop educational resources and initiate health education activities to support individual health and recovery.
- Serve as a preceptor for nurse practitioner students, providing guidance and supervision.
Collaboration & Liaison:
- Collaborate with a LPN’s, Pharmacists, and Physicians, community agencies, and stakeholders to enhance care and coordinate services.
- Coordinate with community organizations to ensure comprehensive care and support for individuals.
Cultural Competence & Practice:
- Integrate Indigenous cultural perspectives and trauma-informed, recovery-oriented approaches into practice.
- Consider the psychosocial, emotional, ethnic, cultural, and spiritual dimensions of individual health.

- Engage in activities to improve healthcare support efficiencies and individual outcomes.

Qualifications:
- Completion of a recognized Nurse Practitioner Program at the Master of Nursing level or equivalent.
- Active registration with the College of Registered Nurses of Alberta (CRNA) in the Family/All Ages Stream.
- Basic Cardiac Life Support (BCLS-HCP), Advanced Cardiac Life Support (ACLS), Basic Trauma Life Support (BTLS). Additional advanced clinical courses preferred.
- Experience in assessment, treatment, therapeutic interventions, and community-based care, especially with mental health and substance use challenges.
- Certification or relevant training in Opioid Agonist Therapy (OAT) preferred.
- Proficiency with electronic medical records and related systems, security clearance, and experience with diverse populations.
