Position Summary:
The Registered Practical Nurse (RPN), reporting to the Manager of Hospice Care, will provide day-to-day nursing care to residents by assessing, planning, implementing, and evaluating their care needs within the scope of practice of an RPN. The RPN will work alongside the Registered Nurse (RN) and collaborate with the interdisciplinary team, providing support and direction to the Personal Support Workers (PSWs) and Hospice Volunteers. The RPN will liaise with other health care providers to ensure a resident and family-centred plan of care is maintained. All duties will be performed in compliance with the Occupational Health and Safety Act, related regulations, and Barrie Family Hospice of Elgin’s Policies and Procedures.
Key accountabilities of this role include:
Responsibilities:
· Provide nursing care consistent with the RPN scope of practice as defined by the Regulated Health Professions Act and the College of Nurses of Ontario Standards.
· Assist the RN on assigned shifts by supporting resident care assignments, workload and planning.
· Communicate clearly and concisely regarding resident status and care needs with the interdisciplinary team.
Assessment:
· Use evidence-based tools and clinical judgment to assess pain and symptom management needs of individuals and families within the RPN scope.
· Recognize and respond to changes in health status, escalating to the RN or other medical professionals as required.
· Report and summarize clinical findings during team and shift reports.
Planning:
· Contribute to the development and updating of individualized care plans under the guidance or in collaboration with the RN and other care providers.
· Consult with the individual, family members and/or the substitute decision maker regarding needs, wishes and preferences for care.
· Consult with the Ontario Health at Home regarding additional services as required for the care plan.
Implementation:
· Administer medications and treatments as ordered, adhering to safe medication administration practices and Hospice policies.
· Implement medical interventions within the RPN scope, including the use of medical supplies and equipment.
· Document accurately and in a timely manner according to the College of Nurses of Ontario Standards and Hospice of Elgin policy.
· Order and maintain medical supplies according to the individual’s needs in collaboration with the Ontario Health at Home Care Coordinator
· Identify and report unusual incidents in the delivery of care in the Hospice to the RN and Manager of Hospice Care in a timely manner.
· Delegate and /or teach skills and techniques to other staff and /or individuals’ families in compliance with the Regulated Health Professionals Act.
Evaluation:
· Participate in daily review and evaluation of individual care plans, recommending changes as necessary within the RPN scope.
· Evaluate nursing and medical interventions.
· Accurately follow all medication administration practice standards and adhere to Best Practices in the administration of medication.
· Ensuring the accuracy of the safe medication practices by following the medication management policies of Barrie Family Hospice of Elgin.
· Confer with individuals and family regarding satisfaction with the plan of care.
· Assist with the writing of clinical policies and procedures when asked.
· Participate in the College of Nurses of Ontario Quality Assurance program.
· Other duties as assigned.
QUALIFICATIONS:
· Registered Practical Nurse in good standing with the College of Nurses of Ontario.
· Minimum of 3 years' experience working in palliative care settings.
· CAPCE Certificate of Completion or commitment to obtain within 2 years of start date.
· LEAP Certificate of Completion or commitment to obtain within 2 years of start date
· CNA Certification in Hospice Palliative Care preferred.
· Demonstrated experience providing hospice palliative care, with expertise in pain and symptom management and care provision across all Domains of Issues of Illness and Bereavement.
· Ability to manage terminally ill individuals and their families, with the necessary knowledge, skill, and physical capacity.
· Ability to function in a leadership role within an interdisciplinary team environment.
· Demonstrated critical thinking, assessment, and problem-solving skills.
· Demonstrated relationship management skills consistent with the Noble Purpose of Barrie Family Hospice of Elgin.
· Excellent communication and interpersonal skills.
· Demonstrated computer proficiency.
Additional Requirements

- Be available for 12-hour shifts, with a rotating schedule, including weekends and holidays.
Compensation
· $30.22 -$32.22/hour
