About the Reactivation Care Centre (RCC):
Our specialized 16 bed inpatient reactivation program is located on the beautiful and quaint UHN-RCC campus. The SH RCC program is designed for individuals who present with decreased mobility, independence and/or function as a result of an acute illness or surgery. We focus on patients with potential for functional improvement, providing tailored care plans and utilizing state-of-the-art tools to support their recovery journey. Depending on the reactivation goals patients typically experience an average length of stay of 30 to 60 days. A Registered Nurse will complete patient admission assessments and work collaboratively with you to assign patients appropriately. You will collaborate with a high performing multi-disciplinary team including Physicians, Registered Nurses, OT, PT, SW, and SLP to provide high quality and safe patient care.
In this role you will:
- Collaborate with the interdisciplinary team in the establishment of goals and objectives for the program consistent with the provision of high-quality care
- Participate in the programmatic development of the clinical area and participate and provide leadership in nursing practice and evaluation
- Aid in the development and maintenance of standards of clinical practice through the creation of an environment conducive to growth and development of staff, excellence in clinical practice and implementation and utilization of research
- Interpret and foster the philosophy, goals and standards of the Department of Nursing
- Develop partnerships within Sinai Health and within the community at large, including identification of community resources, multi-cultural services, and adult and geriatric services to enhance continuity of care for clients
- Perform and participate in research activities. Implement and advance changes in evidence-based practice
- Deliver, coordinate and participate in education and professional development activities/programs involving both staff and patients
- Perform activities to further develop and strengthen the profession including the supervision of advanced nursing students
- Act as a resource and serves as a consultant to individuals and groups within the professional community
