Telemedicine IMPACT Plus (TIP) Facilitator University Health Network Toronto, ON University Health Network (UHN) is looking for an experienced professional to fill the key role of Telemedicine IMPACT Plus (TIP) Facilitator in our Primary Care Department. The Facilitator, IMPACT PLUS uses in-depth clinical knowledge, educator skills, research and clinical expertise to ensure patients, with complex co-morbid conditions, receive services/ coordinated care in partnership with primary care practitioners in each designated Health Link, with the ultimate goal of preventing avoidable emergency department (ED) visits and hospitalizations. The Facilitator works collaboratively with patients and their caregivers, primary care providers, inter-professional (IP) teams and specifically with Home and Community Care coordinators to ensure that care plans are documented, shared, implemented and monitored.
Responsibilities:
Assists in the identification of patients who meet the criteria to participate in the Telemedicine IMPACT Plus program and performs client intake and assessment responsibilities
Coordinates case conferences, using OTN technology, assists with case presentation, facilitates integrated care planning with patients, their families and Primary Care Providers and determines who will be stewarding the care plan in collaboration with providers
Participates in the orientation, training and education process in this model of care
Supports program development and evaluation and sharing of research and knowledge
Performs cross-functional and/or other duties consistent with the job classification, as assigned or requested;
Working in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention’s of the Act, in a timely manner
Qualifications:
Licensed member of a Regulated Health Profession in good standing with the regulatory body in Ontario
Solid knowledge of health care related legislation and practices and collaborative patient service delivery
Ability to think strategically to provide the best health care services for eligible patients and alternatives for those ineligible
Experience in geriatrics/primary care/community health is an asset
Demonstrated in-depth expertise regarding care of vulnerable clients, including those with complex co-morbidities and chronic mental health challenges
Sound knowledge of and skill with adult education techniques Highly developed information technology and computer skills
Experience with telemedicine use an asset
Case Management Certificate is an asset
Valid Ontario Driver’s License
