Established in 1969, REACH is a non-profit, community-governed organization with a history of primary care innovation and a commitment to equality and social justice in East Vancouver. We have a multidisciplinary approach to care within our medical, dental, pharmacy, multi-cultural family centre and an Urgent Primary Care Centre (UPCC).
Summary of Position:
The primary care nurse cares for families and individuals across the lifespan. This includes health promotion, screening and assessment, counselling and education, and treatment and for minor and acute illnesses, and chronic care management within the nursing scope of practice. The primary care nurse is expected to be able to work independently and work within the standards, limits, and conditions determined by relevant legislation and BCCNM; and the limits of the nurse's individual expertise. The nurse is expected to consult with a doctor when appropriate. It is important that the primary care nurse be willing and interested in working collaboratively as part of the interdisciplinary team at REACH; be committed to the goal of improving the health of people living in the community; and adhere to the REACH code of ethics. The nurse reports to the operations manager and is to follow protocols and procedures as set out by the operations manager. The nurse will always act within scope of practice as set out by the BCCNM.
Summary of Responsibilities:
- Provide limited supportive counseling for patients in crisis; referrals to appropriate community resources
- Document any observations, assessments, diagnostics, interventions, outcomes, communications, along with information about referrals according to our practice procedures and standards using OSCAR EMR.
- Participate in care conferences with the team, and staff and committee meetings.
- RN will consult with a doctor at any point in the patient's care when the nurse approaches their limits of scope of practice or individual competence.
Qualifications and Skills:
- Completion of a recognized Registered Nurse education program
- Current practicing registration as a Registered Nurse with the British Columbia College of Nursing Professionals (BCCNP)
- Significant nursing experience (minimum five years) with recent and related work in a variety of settings including primary care and/or ER
- Current CPR-HCP Certification essential.
Additional Skills
- Ability to work both independently and collaboratively within an interdisciplinary team.
- Experience working with marginalized populations (mental health and addictions, poverty, vulnerable youth, LGBTQ2I+, and other community members) desirable
- Demonstrates a commitment to the social determinants of health being the foundation for providing care.
- Phlebotomy and IV insertion skills or willingness to develop these competencies.
- Demonstrated skill in provision of evidence based, ethical, culturally safe and appropriate care to all patients
- Knowledge of community and mental health resources an asset
- Ability to communicate effectively with patients, co-workers and community.
- A second language is an asset
If you, or your relative, is a current patient of REACH medical, and are successfully hired for a position in the centre, you may not use REACH for ongoing primary care medical services.
REACH will assist any employee or family member who loses their primary medical care as a result of this policy to obtain comparable medical care as soon as possible after the conclusion of the probationary period.
