Overview
This position reports to the Professional Practice Manager and will be primarily responsible to support outpatient Diabetes,Metabolic Health, & Extensive Needs Services clinics. This position may have additional responsibilities of assisting with otherareas within the hospital as required and may include off-site transitional care unit coverage one day per week as needed
The Registered Dietitian is an integral part of the inter professional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The Registered Dietitian will deliver comprehensive diabetes education, nutritional therapy, education and prevention to clients for the management of T1 and T2 diabetes in adults, gestational diabetes and cardiometabolic and obesity care. The Registered Dietitian will also provide services within the Extensive Needs Services clinic to provide nutritional assessment and intervention to paediatric patients with multiple complex neurodevelopmental, behavioural, medical and physical needs, as well as social vulnerabilities, as well as their caregivers. In addition, the Registered Dietitian may provide ongoing assessment, and nutritional care planning for the transitional care unit for patients. The successful candidate promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. The Registered Dietitian will be an expert in diabetes education, medical nutrition therapy, and will have specialized in the nutrition care of adult patients. The dietitian will work collaboratively with the interdisciplinary team to provide guidance to clients controlled with diet or insulin by providing suggestions to improve diet choices and suggest changes to insulin doses for candidates on insulin injections or insulin pumps. The candidate will identify nutrition related patient educational needs for the diabetes area; initiates, prepares and presents educational programs and materials in collaboration with departmental, program and other team members. The successful candidate will act as a resource person in diabetes for other dietitians and the interdisciplinary team by providing support forhandling complex cases within the acute care setting. The candidate will also participate in the education of dietetic and other students as applicable.
Education
- University Bachelor's Degree in Nutrition or recognized equivalent required.
- Registration in good standing with the College of Dietitians of Ontario required.
- Registration with the Dietitians of Canada preferred.
- Successful completion of dietetic internship program accredited by EQUAL required.
- Certified diabetes educator certificate preferred.
Qualifications
- The successful candidate will have a minimum of 2-years recent clinical experience and expertise in diabetes management and education in a hospital setting with a focus in specialized and complex diabetes care preferred (e.g., gestational diabetes GDMclinic, Type 1 Diabetes, complex type 2, Insulin Pump Management).
- Minimum 2 years experience working in an acute, hospital setting
- Experience working with patients with metabolic conditions and bariatric conditions to optimize diet and weight before and after surgical intervention preferred
- Experience in paediatric nutritional care preferred
- Certified Insulin Pump Trainer preferred
- Demonstrated ability to provide collaboration and consultation with inter-professional team and communityresources.
- Demonstrated knowledge in person-centered care approach and principles
- Demonstrated ability to apply adult learning principles to patient and family education
- Demonstrated ability to deliver education to various groups including other health professionals, students, patients/ families, colleagues and community agencies.
- Demonstrated ability to prioritize and work effectively in a fast paced environment
- Ability to work effectively as a member of the interprofessional team
- Experience providing patient/family education and counselling
- Demonstrated initiative, innovative thinking, teamwork and ability to effectively prioritize work
- Demonstrated application of evidence-based practice
- Demonstrated commitment to ongoing professional development- Computer skills (Microsoft applications).
