Overview:
The Diabetes & Seniors Programming Coordinator is responsible for promoting community health and cultural well-being by providing targeted diabetes education, running wellness support groups, and delivering nutritious, diet-conscious meals. The Coordinator will be responsible for successfully planning and facilitating community and senior-focused events, managing program budgets, and fostering positive community relationships.
Key Duties and Responsibilities:
- Coordinate, organize, and facilitate awareness workshops, events, and activities that promote healthy families, cultural awareness, and increase understanding of diabetes in the community.
- Manage financial program expenses monthly for efficient use of funds, accurate recording, and timely reporting by submitting all receipts and required forms for approval.
- Follow funding agreement requirements and guidelines for the Meals on Wheels, Social, and Congregate programs to ensure their continued success in meeting community needs.
- Prepare daily meals for the Meals on Wheels program following Safe Food Handling guidelines, package them, and deliver them safely to approved clients using a company vehicle.
- Prepare for and host a wellness group to provide support, information, and resources for community members dealing with diabetes.
- Budget and shop for weekly groceries, and oversee any kitchen volunteers or staff assisting with food preparation to ensure health, safety, and recipe compliance.
- Maintain detailed, confidential client case files, tracking appointment metrics, total visits, meals delivered, and participant statistics for reporting and analytical purposes.
- Coordinate and advertise senior-focused events, workshops, monthly shopping trips, and large community functions (such as the annual Christmas Lunch).
- Collaborate with all Wahta employees, departments, and external service providers, including assisting Family Well-Being with after-school programs.
- Encourage the learning and use of the Mohawk language and support the organization's dedication to cultural advancement.
- Protect the privacy and confidentiality of community members and staff by strictly safeguarding client and sensitive information.
- Conduct other related duties and relevant tasks as level as assigned by leadership.
Skills Required:
- Proficient level of communication skills (verbal and written) with a strong comfort level dealing directly with the public, managing complaints politely, and practicing effective listening.
- Strong computer skills, including data entry, file management, and standard Microsoft Office applications.
- Proven capability in financial planning, monitoring annual program budgets, and forecasting expenditures.
- Ability to work effectively and contribute to a harmonious, cooperative team environment.
- Strong problem-solving, organizational, and multitasking skills with the capacity to take initiative.
- Physical capacity to manage regular standing, walking, bending, crouching, and lifting/carrying objects up to 35 lbs (such as loading and unloading groceries).
- Knowledge of Kanienkeha:ka culture and history is a strong asset.
- Must possess a current and valid Driver's License and a dependable vehicle.
Education and Experience Requirements:
- Minimum Certificate in a related field (Social Work, Personal Support Worker (PSW)), or a Grade 12/CAEC coupled with an equivalent combination of education and experience.
- Minimum of 2 years of experience in the food service industry and community support services.
- Indigenous work experience is an asset.
Terms of Employment:
This is a full-time position. The regular schedule is Monday to Thursday, 8:00am – 4:30pm, and Fridays, 8:00am – 2:00pm (37.5 hours per week) however, evening and weekend work will be required occasionally for program delivery and operations. The salary range for this role is $46,900.00 to $70,100.00 annually.
