POSITION:
In this position, the Pharmacist provides comprehensive pharmacy services to ambulatory chemotherapy/systemic therapy patients and provides centralized pharmacy services to inpatients within the PM Cancer Centre Program. As a member of the Patient Care Team, the duties and responsibilities include: review, check, and dispense all prescribers' ambulatory standard-of-care and clinical trials oncology medication orders for appropriateness; identify, resolve and prevent real or potential drug therapy problems; monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines; conduct medication reconciliation or patient counselling on a consult/ referral basis; follow up on medication incidents and promote safe medication practices on the unit; respond to drug information questions; providing direct and/or functional supervision of technician staff; performing other duties consistent with the job classification, as required. In addition, you will facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values of caring, excellence, teamwork, innovation, integrity, leadership, and respect.
QUALIFICATIONS:
Education:
Completion of university degree (Bachelor's of Science Degree in Pharmacy or PharmD), or equivalent required.
Certificate of competency from the Pharmacy Examination Board of Canada required.
Active registration with the Ontario College of Pharmacists (Part A) required.
Experience:
Hospital pharmacy practice experience in oncology preferred
Hospital pharmacy residency an asset.
Membership in the Canadian Society of Hospital Pharmacists an asset.
