Unit Summary

Medicine Overflow is an active adult general internal medicine unit. An Interdisciplinary team provides care to an acutely ill,
primarily geriatric patient population; including patients with multi system chronic illnesses.

Position Summary

As a member of the interdisciplinary health care team, the Social Worker provides direct patient and family care through the provision of psycho-social screening and assessments/ individual and family counseling; crisis intervention; bereavement counseling; liaison and referral to community agencies; advocacy; case management and discharge planning. The Social Worker collaborates with other team members, participates in program development, and participates in professional education and research.

Schedule Work Hours

Mondays to Fridays; days

This position is temporary until September 2020.


1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a SocialWorker
2. Master's degree in Social Work from an accredited School of Social Work preferred
3. Recent (3-5) years experience providing services in an acute care environment
4. Demonstrated knowledge of Regional community support services
5. Working knowledge of the Healthcare Consent Act and Substitute Decisions Act
6. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case
management, crisis intervention and advocacy skills
7. Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem-solve effectively
8. Strong interpersonal, communication, teamwork, consultation and teaching skills
9. Demonstrated commitment to ongoing learning of self and others
10. Qualified as a field instructor for social work students and demonstrated experience supervising students
