Reporting to the Housing Manager, the Outreach Worker participates in the recovery and capacity building of residents and alumni to maximize their social and economic potential and support their goals for successful independent living in the community.

Advocate for residents who are within 2 months of completion of a publicly-funded residential addiction treatment program. This will include Phoenix Society Programs and other publicly funded programs in the Fraser Valley region.
Discharge planning services for individuals being released from publicly-funded residential addiction treatment programming (within 2 months).
Help sustain tenancies by assisting clients in recovery to find and secure affordable housing.
Ensure ongoing assessment of client progress regarding levels of support needed.
Support individuals through crisis including tenancy breakdown.
Provide 3rd party mediation by facilitating a discussion that focuses on clear communication and achievable solutions between clients and landlords.
Build and maintain relationships with landlords.
Provide support for clients to develop life skills and support community engagement.
Offer assistance for rental applications as well as other required documentation such as applying for personal identification, healthcare requests, training, jobs, etc.
Establish and maintain appropriate community partnerships.
Ensure that client files are maintained and current within established guidelines.
Two year diploma in social work or an equivalent field of study
At least one year work experience in a related field
Valid Class 5 driver’s license with driver’s abstract
Ability to apply trauma-informed practices to clinical care
Strong collaborative approach to delivering and developing programs and services
Proven communication and conflict resolution skills
Effectively communicate professionally, verbally and written in English
Current Tuberculosis Screening Certificate
Office suite skills including Excel, Word, Outlook and central data systems
Current First Aid/CPR
Criminal Records check for working with vulnerable populations
An equivalent level of experience and education may be considered
