Overview
We are seeking compassionate and dedicated Health Care Assistants to join our team in providing exceptional care to clients in their home care settings. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and the ability to work effectively with seniors, individuals with disabilities, and those experiencing dementia or hospice care needs. This role offers an opportunity to make a meaningful difference in the lives of our clients by delivering personalized and respectful care.
Duties
- Assist residents with activities of daily living, including bathing, dressing, grooming, and mobility support
- Post-hospital discharge care, respite care and End of life care.
- Support patients with meal preparation and feeding as needed
- Monitor and document patient conditions, reporting any changes to healthcare professionals
- Provide companionship and emotional support to residents, fostering a caring environment
- Maintain cleanliness of patient areas through light cleaning duties to ensure a safe and hygienic environment
- Assist with case management tasks including coordination of services and documentation of care plans
- Support individuals with disabilities by adapting activities to meet their specific needs
- Participate in training related to dementia care, hospice care, and other specialized areas of caregiving
Requirements
- Prior experience in nursing homes, senior care, or home care settings is highly preferred
- Experience working with individuals with disabilities, dementia, or in hospice care is advantageous
- Ability to perform heavy lifting safely when required for patient mobility assistance or equipment handling
- Strong caregiving skills with a compassionate approach to patient care
- Ability to work independently and as part of a multidisciplinary team
- Excellent communication skills for effective interaction with residents, families, and healthcare professionals
- Certification in caregiving or relevant health care training is a plus but not mandatory
