The Opportunity
The Nurse Practitioner (NP) at AgeCare works collaboratively with the Director of Care and an interdisciplinary team to provide quality, safe resident care. The NP promotes a culture of trust, respect, teamwork, and quality, ensuring the rights and individuality of residents, families, and staff are upheld.
What You Will Do
- Collaborate with the MD, DOC, and care team.
- Assess, diagnose, and manage resident health conditions.
- Advocate for residents and engage with families and substitute decision makers.
- Participate in resident care conferences.
- Lead clinical team in care activities, including hospital discharges, palliative care, and dementia management.
- Identify and address trends in resident care, guiding educational programming.
- Promote best practices and mentorship among clinical staff.
- Develop and deliver education to staff, residents, and families.
- Document clinical data and care plans promptly.
- Support a safe and quality-focused organizational environment.
- Participate in Quality Committee meetings and CQI initiatives.
- Provide after-hours and on-call coverage.
What You Bring
- Minimum one year of experience in geriatric, rehabilitation, or long-term care nursing.
- Leadership experience is an asset.
- Certifications in gerontology, mental health, behavioral support, or palliative care are an asset.
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class and entitled to practice.
- Knowledge of relevant Long Term Care healthcare acts and standards.
- Availability for on-site presence.
