As a Patient Care Assistant, you'll play an essential role in supporting patients and families while working alongside nurses, rehabilitation professionals, and a multidisciplinary healthcare team. Every day offers the opportunity to help patients achieve greater comfort, dignity, independence, and quality of life.
The Transitional Care Unit (TCU), located at the Alternative Healthcare Facility in Amherstview, is a 120-bed program supporting patients who require ongoing care while awaiting their next stage of care. Patients may be:

Awaiting long-term care placement

Working toward increased independence through rehabilitation

Receiving palliative and comfort-focused care

As a Patient Care Assistant, you will:

Assist patients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility

Support patient comfort, safety, and independence
-
Assist with nutrition, hydration, and meal support
-
Observe and communicate changes in patient condition to nursing staff
-
Document care activities accurately and according to policy

Help maintain a safe, organized, and welcoming patient care environment

Collaborate with nurses and the interdisciplinary team to deliver exceptional patient-centered care

Requirements:

Graduate of a Personal Support Worker program (or equivalent) within the past two years; or one year of full-time equivalent experience within the past five years

Secondary school diploma or equivalent

Minimum 150 hours of PSW (or equivalent) experience through employment or clinical placement in a long-term care or complex continuing care setting within the past five years

Ability to communicate effectively in English

Satisfactory Criminal Record Check and Vulnerable Sector Screening
