The Opportunity

We’re looking for a Health Navigator. This role will coordinate the highest standard of care services provided by the care team and will assist in maintaining a safe and secure environment for residents, their families’ and other staff.

What You’ll Do

Assess, plan, and evaluate individualized care for residents regularly.

Coordinate resident assessments and service plans.

Liaise with healthcare professionals and maintain accurate records.

Respond to health emergencies and ensure compliance with safety policies.

Coach Care Services team members.

What You’ll Bring

RN or RPN designation with current registration.

Knowledge of seniors’ retirement housing and relevant legislation.

Strong communication and organizational skills.

CPR & First Aid Certification.
