The Position
We are seeking a Case Manager Lead to join our Case Management team at Mercy House. In this role, you will work with individuals facing complex challenges and requiring a higher level of support. Referrals will come from fellow members of the Case Management team, including the Clinical Manager, and your focus will be on coordinating care and services for Guests with greater needs. Reporting to the Manager of Guest Services, your key responsibilities will include:
Key Responsibilities:
- Work closely with the Manager of Guest Services to ensure the efficient running of Case Management Services
- In Consultation with the Manager of Services provide regular updates on complex cases.
- Work with the Manager of Guest Support to assign cases and follow up with Case Managers on outcomes.
- Assign Case Managers when necessary to manage Income Support Clinic, Dr.’s Clinic, etc. and maintain respectful working relationships with said organizations.
- Bring to the attention of the Manager of Guest Services any cases requiring multi-agency input and Case Conferencing.
- Be the first point of contact when issues arise with Case Managers. If issues cannot be resolved at this level bring it to the attention of the Manager of Guest Services.
- Engage with individuals experiencing complex vulnerabilities, as well as barriers to service/care.
- Actively work to remove or lower barriers for guests seeking services, using a harm reduction/trauma informed approach.
- Coordinate service delivery with other relevant providers and link guests with appropriate service in the community.
- Navigate systems alongside guests, taking on an advocacy role as required.
- Build and maintain relationships with Guests, while maintaining clear and consistent boundaries.
- Work as a collaborative member of TGP’s clinical and guest services team and participates as part of a larger multi-disciplinary team when necessary.
- Engage in de-escalation, crisis and safety planning with guests who may exhibit behaviours that preset as barriers to services.
- Responsible for the safe, effective and efficient delivery of services to The Gather Place Guests. Application of organizational policies and processes pertaining to guests and workplace safety is expected.
- Complete outreach visits as necessary.
- Other duties as required.
The Case Manager Lead plays a crucial role in delivering safe, effective, and efficient services to Mercy House Guests while adhering to organizational policies related to guest and workplace safety.
What You’ll Bring
Qualifications and Skills:
- Degree is Social Work, with eligibility for registration with NLASW
- Minimum of two years of experience working with mental health and addiction populations.
- Strong interpersonal skills, ability to creatively build relationships with those who may be socially isolated.
- Sound relationship-building abilities and skills in individual support, conflict resolution, harm reduction and empathetic listening.
- Sound knowledge of available and relevant community resources for marginalized persons.
- Proficient and professional documentation.
- Valid Drivers License and use of vehicle
- Clear Record of Conduct and Vulnerable Sector Check.

This is a full-time permanent position. The Gathering Place – Mercy House offers health and dental, long term disability insurance, life insurance, matching RRSP plan, sick leave, and three week’s paid vacation.
