Addiction Services Central Ontario is seeking a highly motivated and energetic individual to work as a Nurse Practitioner in our Addictions Withdrawal Management Program. This position will be actively involved in offering pharmacotherapy services through our Rapid Access Addictions Medicine *RAAM) Clinics in hospitals and in the community, offer community withdrawal management services to our community partners and in our community clinics, collaborate with community partners to improve quality services through the provision of pharmacotherapy services in the community, provide consultation services to primary care providers in the community and collaborate to develop and implement outreach services to primary care providers, community organizations, hospitals and other organizations, as required. This position will collaborate with our Nursing Team to ensure the provision of the best quality care possible for clients in need and communities. As an active partner within several Ontario Health Teams, ASCO is involved in the development and/or implementation of programs and services. This position will support programs and services offered through an OHT, as determined by the agency.
This position will offer services throughout York Region, South Simcoe and North York, as determined by the agency, and will work closely with the agency’s partners, as requested, to assist the agency in achieving the goals set out by the agency, OHTs and/or its funders. This position works collaboratively to initiate and monitor individuals looking for assistance with opioid use disorder through the initiation and monitoring of opioid agonist therapy (Buprenorphine) as well as provide medical support for individuals who wish to withdraw from alcohol and/or other substances. Working collaboratively with other members of the Addictions Withdrawal Management Team, the Nurse Practitioner will initiate and monitor alcohol anti-craving medications and play a key role in assisting individuals to develop a personalized plan toward stabilization, harm reduction and recovery. The Nurse Practitioner will promote overall harm reduction strategies, including dispensing Naloxone kits.
This position will collaborate with the agency’s Nurse Practitioners team to provide nursing leadership and management to the agency’s nursing team, provide clinical supervision to the nursing team (individual and group supervision) as needed, and work collaboratively with the agency’s Leadership Team to develop, implement and evaluate the agency’s programs and services.
This position will be work throughout York Region. South Simcoe and North York and will participate in the Mobile Withdrawal Management program soon to be established by ASCO.
ASCO is required to follow all public health and provincial directives as well as of those of our Ontario Health Team partners as related to the pandemic and other health issues that may come forward, including but not limited to vaccination requirements. Proof of vaccination will be required prior to being offered a position at ASCO.
This is a full-time position for 35 hours per week., with a minimum of one evening per week, providing services throughout York Region, North York and South Simcoe. Weekend work is also expected (i.e. some Saturdays). The annual salary for this position is $117,000.

Duties and Responsibilities include but are not limited to:

Clinical Services:
-
Perform assessment, diagnosis, treatment, health education and other medical functions based on appropriate College of Nurses’ Standards of Practice
-
Prescribe and monitor withdrawal management, substitution therapy and anti-craving medications for a variety of substance use disorders.

Facilitate access to on and off-site addictions support staff and interdisciplinary team and programs, as needed
-
Work collaboratively with the addictions practitioners and all referring sources
-
Provide education and clinical support to primary care providers to scale up expertise in addictions management organizationally
-
Bed-Based Addiction Withdrawal Management Services:
-
support community Primary Care Providers (PCP) who are providing care to individuals preparing to enter into a bed-based addictions treatment service
-
Mobile Withdrawal Management Services
-
Collaborate with the agency’s Nurse Practitioners to develop and implement a suite of mobile withdrawal management services for York Region, North York and South Simcoe
-
work closely with the agency’s partners, as determined by the agency, to offer withdrawal management services at or near their locations
-
Quality Improvement – Addiction Services
-
Collaborate with Wellness Collaborative partners in the delivery of quality improvement initiatives for bed-based addiction services
-
Rapid Access Addictions Medicine (RAAM) Clinics and Community Clinics
-
Collaborate with the inter-disciplinary team to serve individuals access RAAM Clinics and/or Community Clinics
-

Support the agency’s nursing team, and provide the nursing staff with clinical and administrative supervision, both individual and group, as needed

Collaborate with the Nurse Practitioners’ Team to provide appropriate clinical training to the Nursing Team

Collaborate with the Leadership Team to provide appropriate clinical training to agency front-line staff
-
Provide primary care support to individuals who are seeking pharmacotherapy for their substance use, as required.

Work in a manner that is mindful of the whole person/community within a social context and being cognizant of the impacts of the social determinants of health

Work sensitively with equity seeking groups who wish to access addictions withdrawal management programs and services

Ensure clinical records are maintained, accurate and up to date on a consistent basis

Qualifications:
-
Valid Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, RN(EC) with a speciality certificate in Primary Health Care
-
Primary Health Care Nurse Practitioner Certificate or Adult Nurse Practitioner Certificate (COUPN or equivalent) required
-
Demonstrated experience working effectively with communities/ individuals who face barriers accessing services and experience in approaching work from an equity lens
-
Demonstrated experience in Opioid/Alcohol Management/Harm Reduction preferred
-
Demonstrated excellent problem-solving, organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment.
-
CNA certification in Psychiatric/Mental Health Nursing would be an asset
-
Proof of liability protection under NPAO (or independently) required
-
2 or more years’ experience in delivery of primary care preferred
-
Clinical experience in the mental health/addictions field would be a definite asset
-
Awareness of and sensitivity to the needs of people with mental illness and/or addiction
-
Excellent clinical skills and sound clinical judgment
-
Excellent organizational and communication (written and oral) skills
-
Strong interpersonal, leadership and team-building skills
-
Solid problem-solving, negotiation and conflict resolution skills

Ability to work independently and as a team member

Experience in policy development, program planning and evaluation would be an asset
-
Proficient in the use of IT systems and computers for word processing, email and data entry (Outlook, Excel, electronic client records)

Experience with OTN and telemedicine would be an asset

Comfortable providing virtual care, as required

Flexibility, tolerance for ambiguity and a good sense of humour are essential for this position

Excellent English communication skills, both verbal and written

Ability to speak in French, a definite asset

Valid driver’s license and reliable transportation with appropriate insurance coverage
