Key Responsibilities

Provide professional social work and crisis intervention services to seriously mentally ill clients.

Develop and implement creative treatment and recovery plans, discharge planning, support services and programs responsive to clients’ needs.

Engage in client-centered care.

Collaborate with the inter-professional team.

Maintain appropriate documentation and data collection, in accordance with hospital, college and legislative standards.

Participate in the development and delivery of educational and information sessions for clients, families, service providers, and the community.

Participate in Social Work meetings, peer consultation and committees.

Keep up to date regarding professional standards by reviewing literature, reading articles, attending educational events and networking with colleagues

To work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the ROHCG.

To ensure a work environment that is conducive to the ROHCG Harassment Free policy.
Qualifications

Bachelor of Social Work from an accredited university.

Three years’ experience in a mental health setting.

Member in good standing with the Ontario College of Social Workers and Social Service Workers.

Knowledge, skills and training in concurrent disorders.

High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.

Knowledge of community resources, corrections, and applicable legislated acts (MHA, PHIPA, The Health Care Consent Act, etc.).

Understanding of recovery-based treatment and programs of clients with severe and persistent mental illness.

Demonstrate ability to work both independently and in a professional inter-disciplinary team environment.

Strong advocacy & conflict resolutions skills.

Strong organizational skills and work ethic, with excellent oral and written communication skills.
Language Requirements
English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression.
Salary Range:
43.31 to 48.72 per hour
