We invite compassionate, motivated individuals to apply for the position of Community Connection Worker, a role that plays a vital part in strengthening access to culturally safe supports and enhancing connections to community and culture.
About the Role
As a Community Connection Worker, you will provide person-centred support that promotes inclusion, independence, and self-determination. You will assist individuals in navigating CLBC systems and community services, strengthening coordinated planning among service providers, and supporting individuals to achieve their personal, social, vocational, and volunteer goals.
This role requires a strong commitment to dignity, cultural safety, collaboration, and high standards of care.
Core Responsibilities
- Increase access to culturally safe and relevant supports
- Strengthen individuals’ connections to culture, community, and natural support networks
- Support individuals in navigating CLBC systems and related community services
- Advocate for the rights, choices, and well-being of the individuals you support
- Build respectful, meaningful relationships that empower individuals to participate fully in community life
- Facilitate involvement in community-based activities aligned with individual goals and interests
- Collaborate with families, CLBC representatives, professionals, and community partners to enhance coordinated service planning
- Maintain accurate documentation, including progress notes and individual support plans
- Apply appropriate behaviour-support and management strategies for individuals with diverse needs
Qualifications & Requirements
- Grade 12 education required; Community Support Worker Diploma or equivalent preferred
- Minimum of two (2) years’ experience in community social services, healthcare, or a related field
- Experience supporting individuals with diverse abilities and navigating community-based systems (CLBC experience considered an asset)
- Current First Aid and CPR certification; Food Safe certification required
- Strong verbal, written, and non-verbal communication skills
- Valid BC Driver’s License and access to a reliable vehicle with appropriate insurance
- Clear Criminal Record Check for working with children and vulnerable adults
- Basic computer literacy and legal eligibility to work in Canada
