Position overview
Right at Home Canada Toronto is seeking a motivated, reliable, and clinically confident Registered Nurse (RN) or Registered Practical Nurse (RPN) to provide high-quality care in clients’ homes and other community settings.
Visits are generally short in duration—often approximately one hour or based on the nursing service required. The successful candidate will support both private clients and patients referred through corporate partnerships. Because this contract is centrally coordinated across multiple offices, flexibility, responsiveness, and dependable communication are essential.
Mandatory orientation and training will be provided before corporate visits are assigned. Nurses will also receive the relevant medical information and care instructions for each case. However, applicants must already be competent and confident performing the required clinical procedures independently. Procedure-specific shadowing is not provided.
Key responsibilities
- Provide safe, compassionate, and professional nursing care during short-duration community visits.
- Complete assessments, monitor vital signs, administer medications, and perform assigned clinical procedures within scope of practice.
- Review and follow the client’s care plan, medical information, and case-specific instructions before each visit.
- Recognize and report changes in condition, escalating concerns appropriately.
- Maintain clear communication with clients, families, caregivers, office staff, and members of the healthcare team.
- Complete accurate electronic documentation using the company-issued iPad.
- Follow all infection prevention, privacy, safety, and professional practice standards.
- Travel to private residences and corporate-referred patients throughout the designated service region.
- Accept ongoing assignments, including short-notice and short-duration visits, based on availability.
Required qualifications
- Current RN or RPN registration in good standing with the College of Nurses of Ontario (CNO).
- Current CPR and First Aid certification.
- Current Vulnerable Sector Screening (VSS) or acceptable criminal record check.
- Current two-step TB test.
- Valid driver’s licence and reliable access to a vehicle; public transit is not suitable for this role.
- Ability and willingness to travel throughout the designated region.
- Strong clinical assessment, critical-thinking, and decision-making skills.
- Ability to work independently in a home-care or community setting.
- Strong communication, time-management, and documentation skills.
- Comfort using technology and completing electronic visit notes on an iPad.
- Reliability, professionalism, responsiveness, and flexibility.
- Previous home care, acute care, assisted living, long-term care, pediatric, senior care, or dementia care experience is an asset.
- Familiarity with electronic health record systems such as Epic or Cerner is an asset.
Required clinical competencies
Applicants should be independently competent in most of the following skills. RPN applicants must meet the majority of the clinical skills checklist:
- Intramuscular injections: Administer medications by the IM route.
- Subcutaneous injections: Administer medications by the subcutaneous route.
- Capillary blood glucose monitoring: Use a glucometer, check blood glucose, and provide related health teaching.
- Insulin pens: Prepare and administer insulin using insulin pens.
- PICC management: Assess lines, change dressings, administer medication, flush, and lock.
- Peripheral IV management: Assess lines, change dressings, administer medication, and flush.
- Port-a-Cath management: Access ports, change dressings, administer medication, flush, and lock.
- CADD Solis pump: Operate and manage the CADD Solis pump.
- Elastomeric pump: Operate and manage elastomeric infusion pumps.
- IV gravity therapy: Initiate and manage IV therapy by gravity.
- General wound care: Complete wound assessments, dressing changes, irrigation, and packing.
- Suture and staple removal: Safely assess and remove sutures or staples.
- VAC therapy: Operate VAC equipment and remove and apply dressings.
- Enteral feeding: Administer feeds and manage feeding tubes and insertion sites.
- Kangaroo pump: Operate and manage Kangaroo pumps for enteral feeding.
- Chest tube management: Assess chest tubes and complete related dressing changes.
- Chest drainage systems: Manage Pneumostat, Atrium Mini, Heimlich valve, and P-Eggy drainage systems.
- Tracheostomy management: Provide general tracheostomy care, including suctioning when ordered.
- T-tube and laryngectomy care: Provide laryngectomy care and manage T-tubes.
- Oxygen therapy: Manage oxygen therapy and titrate oxygen according to orders.
- Stoma care: Manage ileostomies and colostomies.
- JP drains: Provide general management and remove JP drains when ordered.
- Abscess drains: Manage and flush drains and complete dressing changes.
- PTC drains: Manage and flush drains and complete dressing changes.
- Tenckhoff catheter: Provide general catheter management.
- Foley catheter: Perform catheterization and provide ongoing catheter care.
- Phlebotomy and specimen collection: Collect blood and other laboratory specimens using safe techniques.
- Medication administration: Administer medications safely and accurately within scope of practice.
- Pain and symptom management: Assess symptoms, implement the care plan, and report concerns.
Scheduling and work requirements
- Consistent commitment of 20 hours per week.
- Initial three-month contract with potential for a permanent and full-time opportunity.
- Short-duration visits, commonly approximately one hour or based on the nursing service required.
- Availability for ongoing assignments and occasional short-notice visits.
- Travel to private and corporate-referred clients across the designated region.
- In-person community-based work.
