Midaynta Community Services is a Toronto-based community organization dedicated to improving the quality of life for newcomers, refugees, youth, families, and communities experiencing systemic barriers. Through culturally responsive programming, advocacy, capacity building, and community partnerships, Midaynta supports housing stabilization, settlement, youth empowerment, family well-being, and community engagement.
Position Summary
The EPIC Case Manager provides rapid, short-term, person-centred case management to tenants who are at imminent risk of eviction or homelessness. Working collaboratively with tenants, landlords, housing providers, legal services, income supports, and community partners, the Case Manager develops immediate intervention and housing-stabilization plans designed to preserve and stabilize tenancies wherever possible.
The role includes home visits, assessment, advocacy, landlord negotiation, service coordination, and urgent intervention. Each referral is unique, complex, and time-sensitive. The Case Manager develops comprehensive case and sustainability plans and maintains timely records in SMIS, TGRIP, and other City of Toronto case-management systems.
Rapid Response Expectations
Upon receiving a referral from the service queue, the Case Manager is expected to contact the client/household within 48 hours to review the situation and develop a plan of action collaboratively. Urgent matters, including cases involving a sheriff’s order, require immediate response and take precedence.
Key ResponsibilitiesCase Management & Crisis Intervention
· Provide responsive, person-centred case management tailored to each household’s unique needs and circumstances.
· Screen, assess, and intake clients and develop individualized action plans for housing stability.
· Conduct home visits, accompany clients to service providers, and work flexible hours when necessary.
· Implement rapid, wraparound, short-term interventions to prevent eviction and sustain tenancies.
· Provide non-judgmental, trauma-informed, anti-oppressive services grounded in harm reduction and the Housing First approach.
Eviction Prevention & Housing Stabilization
· Build and maintain relationships with housing providers, including landlord outreach and engagement.
· Mediate and negotiate with landlords to preserve tenancies wherever possible.
· Educate clients about their rights and responsibilities under the Residential Tenancies Act and other housing-related legislation.
· Connect clients with rental-arrears assistance, legal supports, income programs, and other community resources.
· Develop sustainability plans and provide short-term follow-up before discharge through Coordinated Access.
· Refer clients to Local Access Centres and Housing Help Centres when relocation is necessary.
Systems Navigation & Community Resources
· Maintain working knowledge of the Rent Bank, Eviction Prevention Fund, Housing Stabilization Fund (HSF), Landlord and Tenant Board processes, Ontario Works (OW), Ontario Disability Support Program (ODSP), and municipal housing policies.
· Coordinate with legal clinics, income and employment programs, mental-health and addiction services, and other providers connected to the client/household.
· Attend City of Toronto meetings and training as required.
Documentation & Accountability
· Maintain accurate and timely daily and monthly case records in SMIS, TGRIP, and other required City of Toronto systems.
· Maintain client appointments and case notes and provide required updates to program management.
· Perform other related duties as required.
Qualifications
· Post-secondary degree or diploma in Social Work or a related field, or an equivalent combination of education and relevant experience.
· Minimum three years of experience providing intensive case management to clients with complex needs, including people who have experienced or are at risk of homelessness.
· Strong assessment, case-management, advocacy, mediation, negotiation, problem-solving, and crisis-intervention skills.
· Strong oral and written communication skills.
· Broad understanding of the issues affecting vulnerable tenants and households experiencing housing instability.
· Strong knowledge of Toronto community resources and housing-support services.
· Proficiency in Microsoft Word, Outlook, Excel, and electronic data/reporting systems.
· Ability to work independently and collaboratively while managing multiple urgent and time-sensitive files.
· Ability to travel throughout Toronto to conduct home visits and attend community appointments.
