JOB SUMMARY:
Under the direction of the Director of Care/ Designate assesses, implements and evaluates person-centered care for residents living in Finnish Home. Promotes quality of life, ethical decision-making, collaborative care and professional practice alongside the interdisciplinary team. Acts as the in-charge nurse, conduit to physicians and emergency care, makes referrals to appropriate discipline. Provides support to LPNs for residents who become unstable or are at the end of life. Oversees wound care, pain management and tube feeding practices. Follows professional practice standards as outlined in the BCCNM.
DUTIES & RESPONSIBILITIES:
- Provides direct resident care using the nursing process, which includes assessment, care planning, intervention and evaluation.
- Performs a range of nursing procedures and treatments as required by the resident care plan
- Plans, implements, evaluate and document the individualized resident care plan and ADL
- Participates in the preparation of documents for care conferences by completing appropriate forms and assessments. .
- Attends and participates at care conferences meetings.
- Completes MDS assessments annually and quarterly, using Point Click Care.
- Acts as an advocate to protect and promote resident rights to autonomy, respect, privacy, dignity and access to information.
- Maintains a safe environment for residents, families, visitors and staff by adhering to established safety and emergency practices and procedures. .
- Participates in creating and maintaining a safe and healthy work environment by understanding and following emergency management plans, including codes and infection control.
- Identifies and reports accidents, unusual incidents, unsafe practices, and hazardous unsafe conditions or equipment to the DOC or maintenance department. Completes incident reports as necessary.
- Participates in quality assurance/ improvements and infection control practices to improve resident outcomes.
- Works collaboratively with residents, families, the interdisciplinary team and physicians in the delivery of person-centered care.
- Assists in maintaining the medical supplies by monitoring their use and stock and notifying the DOC/ADOC when supplies require replenishing.
- Completes staff replacement in the absence of the DOC/ Designate (sick calls, emergencies)
- Participates in the orientation of new residents and their families to Finnish Home and the daily routines of the facility. Reports issues and concerns to the DOC.
- Participates in the orientation of direct care staff and students and is responsible for collegially sharing information and demonstrating/ providing guidance on clinical tasks.
- Performs clerical functions that support resident care such as filing documents in residents’ charts, documenting doctor’s orders, answering the phone and responding to issues or referring the caller to the DOC.
- Performs the in-charge role during evenings, weekends and statuatory holidays when assigned.
- Performs other related duties as assigned.
QUALIFICATIONS:
- Graduate of an approved school of nursing, current practicing registration with the BCCNMCPR level C certification and First Aid Level 1
- Ability to communicate effectively in English both verbally and in writing
- Knowledge of clinical geriatric care needs and knowledge of the nursing process in a complex care environment
- Ability to demonstrate and/ or provide guidance to the care provided by care aides in a team environment.
- Ability to deal with others effectively in a team environment.
- Ability to problem solve
- Ability to work as a member of an interdisciplinary team.
- Ability to organize and prioritize work.
- Ability to carry out the physical duties of the position which include lifting transferring and moving residents safely.

Pay: $41.42-$59.52 per hour
