Job Summary
SCH Home Health / Senior Circle Support is seeking reliable and professional Registered Practical Nurses (RPNs) to support our Home Dialysis Assistance Program for clients receiving assisted peritoneal dialysis in the community.
This position is best suited for an RPN who is available for morning and afternoon visits, can provide consistent ongoing coverage, and is comfortable travelling independently between client homes within the Durham Region.
Service Areas May Include but not limited to:
Oshawa, Whitby, Ajax, Pickering, Courtice, Bowmanville, Port Perry, and surrounding Durham Region areas.
Responsibilities
- Attend scheduled visits on time
- Support clients receiving assisted peritoneal dialysis in the community
- Follow client-specific care instructions and agency procedures
- Provide care within the RPN scope of practice
- Complete visit documentation accurately
- Report concerns or changes in client condition promptly
- Communicate professionally with the office team and clinical contacts
- Maintain confidentiality and professional boundaries
Requirements
- Current registration as a Registered Practical Nurse with the College of Nurses of Ontario
- Valid driver’s licence and access to a reliable vehicle
- Ability to travel independently between client homes
- Morning availability required
- Daily or near-daily availability strongly preferred
- Community care or home-care experience is an asset
- Dialysis or peritoneal dialysis experience is an asset, but not required
- Strong communication and documentation skills
- Reliable, punctual, and professional
- Vulnerable Sector Check required or willingness to obtain
- CPR/First Aid preferred
Compensation
$28–$30 per visit
Mileage reimbursement provided where applicable according to company policy
