This position reports to the Care Manager responsible for Children, Youth and Families Addiction and Mental Health (CYF AMH) services across Edmonton Zone community clinics. The Social Worker II works collaboratively with Mental Health Therapists, Addiction Counsellors, Nurses, Psychiatrists, and community partners to support children and youth ages 5-17 and their families who are experiencing mental health, emotional, behavioral, substance use, and complex psychosocial concerns. Working within a patient- and family-centered framework, the Social Worker II provides clinical social work services, supports access to appropriate programs and services, and helps families navigate community, acute care, and specialized mental health systems. The role includes assessment, crisis support, risk assessment and intervention, care coordination, advocacy, and identifying barriers or service gaps that may affect continuity of care. Services are provided using strengths-based, family-centered, culturally responsive, trauma-informed, and concurrent-capable approaches. The Social Worker II contributes to integrated care planning and works collaboratively with clients, families, the interdisciplinary team, and community partners to promote equitable access and coordinated care across the continuum. This position is multi-site and supports clients virtually and in person across all our Edmonton Zone community clinics.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Registration with the Alberta College of Social Workers (ACSW) must be maintained throughout employment. Authorization to perform restricted psychosocial interventions through ACSW is required. Current Basic Cardiac Life Support, Health Care Provider (BCLS-HCP), and Nonviolent Crisis Intervention training. Minimum 3 to 5 years of experience working in child and adolescent mental health, including experience supporting adolescents with complex needs. Experience with crisis management, family work, and supporting children and adolescents with mental health concerns in community settings. Knowledge of DSM-5 diagnoses in children and adolescents, concurrent disorders, and experiential learning. Experience with school and community programming, AHS, community partners, health partnership initiatives, and multicultural groups. Strong clinical judgment and the ability to provide quality care in a changing healthcare environment. Able to work independently across different settings, with strong organizational and time management skills. Comfortable using Microsoft Office, Outlook, and other computer programs. Computer skills may be assessed during the interview. Able to lift and carry medical bags or equipment weighing approximately 12 to 20 kg.
Preferred Qualifications:
Master's degree in social work is preferred. Experience providing care in community settings. Experience Connect Care. Demonstrated ability to work as part of an interdisciplinary team. Excellent interpersonal and communication skills. Training and experience in mental health assessments and therapy. Knowledge of treatment modalities/behavioral interventions for children.
