Catholic Social Services is currently recruiting for a part time (20 hours/week), Support Coordinator CAPCC in Red Deer.
Job Summary:
As Support Coordinator, you will help individuals under the age of 65 living in Facility-Based Continuing Care centers to access community supports to address social isolation and loneliness, removing barriers to community access and social inclusion for the individuals. This position is based out of East/Northeast Central Alberta. The successful candidate will be involved in lots of travel, covering communities such as Camrose, Tofield, Wainwright, Vermillion, Lloydminster, Provost, and Consort.
Duties and Responsibilities:
- Informs and educates continuing care facilities about the CAPCC program
- Participates in community resource building and networking to increase opportunities for community inclusion.
- Supports individuals in-person in their communities.
- Provides community access and recreational opportunities for Individuals outside continuing care facilities
- Provides transportation to individuals to access activities in the community. Transportation provided in staff’s personal vehicles or via public transportation if available.
- Completes intake documentation including needs assessment and risk matrix.
- Creates Individual Support Plans (ISP) annually for individuals that outline the individual’s goals and outcomes related to social inclusion and community access to events and activities; identifies strategies to achieve those goals and outcomes; and identifies what specific supports will be provided to the individual
- Provides information, referrals and advocacy services to other relevant programs and resources
- Assists individuals to identify and or develop formal and informal support networks.
- Supports individuals turning 65 years of age in creating a transition plan 3-6 months prior to final withdrawal of supports.
- Completes electronic file information, quarterly and annual reporting documentation
What This Job Requires:
- Diploma or Degree in Human Services (or equivalent)
- Minimum 3 years’ experience working with individuals with emotional and behavioral needs
- Experience supporting individuals with complex and high behaviours
- Lifting of wheelchairs, walkers, oxygen tanks may be necessary
- Good verbal and written communication is necessary
- Being able to work independently and managing time is essential
- On Call with flexibility to work evenings and weekends when necessary
- Supervisory experience is considered an asset
- Valid First Aid and CPR is considered an asset
- Vulnerable Sector Check
- Child Intervention Record Check
- The position requires driving personal vehicle throughout Central Alberta.
- Transporting clients in personal vehicle is a must.
- Clear and Current Drivers Abstract
- A vehicle and valid class 5 driver's license – requirement to transport clients in your personal vehicle
- Proof of Vehicle Insurance showing $2 million liability coverage
- Proof of Vehicle Registration for insured vehicle
