Passport Support Worker – Adults 18+ | Home & Community-Based
SMITH FALLS and TOLEDO, ONTARIO
We are currently seeking an experienced Support Worker to provide in-home and community-based services to a young lady with developmental disabilities, autism, who will be utilizing Ontario Passport funding. This role supports meaningful community participation, daily living activities, and respite services through a person-centered approach.

Location: Smith Falls and Toledo Ontario
Hours: Flexible short hours (3-4hrs)

Attributes:

Qualifications:
- Completion of a CYW, DSW, CSW, SSW, EA Diploma from a recognized Community College
- Minimum of 1 years' direct work experience in social services with individuals with developmental disabilities (can include placement hours)
- Vulnerable Sector Police Check with Broad Record Check
- CPR and First Aid certification (current)
- Certification in a recognized Crisis Intervention (current NVCI/SMG/UMAB)
- COVID-19 vaccinated
- G class driver's license and access to a reliable vehicle is required
