This is an exciting opportunity to work in the fast-paced environment of the Assertive Community Treatment Team (ACT) and the Adult Community Services program. ACT is a community-based program providing assertive, client-centered, and culturally responsive care to adults experiencing severe and persistent mental illness, addictions, and houselessness, with services primarily delivered through outreach and supplemented by clinic-based supports. Reporting to the Care Manager, this position plays a key role in supporting the day-to-day operations of the multidisciplinary team, including reception and managing patient flow, answering and triaging calls, maintaining electronic medical records (Connect Care), tracking referrals, managing work queues, performing data entry and payroll timekeeping, and ensuring accurate and timely administrative processes. The successful candidate will demonstrate strong organizational, communication, and critical thinking skills, with the ability to manage sensitive information and respond professionally to client concerns, including difficult or urgent situations. Responsibilities also include maintaining awareness of the Mental Health Act and Community Treatment Orders legislation, preparing required documentation, and following appropriate emergency procedures when needed. This role requires strict adherence to confidentiality standards and the ability to build effective working relationships with internal staff and external partners, contributing to a collaborative, responsive, and client-focused care environment.
