Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in our Assisted Living Community. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including seniors, and individuals with dementia or palliative needs. The LPN will play a vital role in supporting patient recovery, managing medication administration, and ensuring a safe and comfortable environment for all patients.
Responsibilities
- Administer medications and treatments accurately according to physician orders
- Monitor patient vital signs and observe for changes in condition
- Assist with patient assessments and document findings in EMR
- Provide wound care, including dressing changes and wound assessments
- Support patients with activities of daily living (ADLs) and mobility assistance
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Educate patients and families about health conditions, medications, and care instructions
- Maintain a clean, safe, and organized environment adhering to infection control protocols
- Document all patient interactions accurately in compliance with healthcare regulations
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing with CLHA
- Prior experience in nursing home, dementia care, home care, hospice care, or senior care preferred
- Knowledge of medical terminology and medication administration protocols
- Experience with electronic health records systems such as Epic or Cerner is advantageous
- Strong communication skills and compassionate bedside manner
- Ability to work effectively both independently and as part of a multidisciplinary team

Pay: $28.77-$40.18 per hour
